摘  要:目的:通过动态观察理筋手法对颈后肌慢性损伤家兔的中枢及外周疼痛调节因子β-内啡肽(β-endorphin,β-EP)、脑啡肽(Enkephalin,ENK)与5-羟色胺(5-hydroxytryptamine,5-HT)表达的影响,探讨在家兔颈部慢性损伤形成的早中晚不同阶段,理筋手法的干预作用及其调节方式。方法:按照随机数字表将63只日本大耳白兔分为正常对照组、模型组、干预组,每组各21只。采用颈椎前屈位固定法建立家兔颈后肌慢性损伤模型,模型组、干预组家兔均采取此法造模,模型组在每天造模结束后不予任何处理。干预组在每天造模结束后给予理筋手法干预,直至60 d观察结束。正常对照组家兔不进行造模干预。每组分别在观察期的第20天(早期)、40天(中期)、60天(晚期)各处死7只家兔。通过HE染色光镜下观察颈后肌肌组织形态学变化,采用ELISA法检测各组家兔中枢(脊髓、下丘脑)β-EP与ENK和外周(颈后肌组织、血浆)中β-EP与5-HT的在各观察点处的含量。结果:HE染色光镜下,模型组及各干预组可见不同程度的肌纤维损害;ELISA检测:理筋手法干预后与模型组第20天、40天相比,干预组脊髓与下丘脑中β-EP、ENK含量有不同程度升高(P<0.05);干预组外周颈后肌组织与血浆β-EP含量升高(P<0.05),5-HT含量降低(P<0.05)。结论:通过早中晚三期的动态观察发现。理筋手法中早期干预能通过调节中枢ENK、β-EP及外周β-EP、5-HT的含量,减轻无菌性炎症对局部组织的炎症刺激,减缓机体对疼痛刺激感受,缓解慢性静力性损伤导致的疼痛。Objective To dynamically observe the central and peripheral pain regulatory factors in rabbits with chronic injury of posterior cervical muscle by LiJing manipulation β-Ep,to investigate the effect of tendon regulating manipulation on the expression of EP,enkephalin(ENK)and 5-hydroxytryptamine(5-HT)in rabbits with chronic neck injury.Methods according to the random number table,63 Japanese white rabbits were divided into normal control group,model group and intervention group,21 rabbits in each group.The chronic injury model of posterior cervical muscle in rabbits was established by cervical anteflexion fixation.The model group and intervention group were established by this method,and the model group was not treated at the end of each day.The intervention group was given LiJing manipulation intervention at the end of modeling every day until the end of observation for 60 days.The rabbits in the normal control group were not intervened.Seven rabbits in each group were killed on the 20 th day(early),40th day(middle)and 60 th day(late)of the observation period.He staining was used to observe the morphological changes of posterior cervical muscle under light microscope,and ELISA was used to detect the central nervous system(spinal cord,hypothalamus)of rabbits in each group β-EP was correlated with ENK and peripheral(posterior cervical muscle tissue,plasma) β-EP and 5-HT were determined.Results HE staining light microscope,different degrees of muscle fiber damage could be seen in the model group and the intervention groups;ELISA detection:compared with the model group 20 days and 40 days after the intervention,there was no significant difference between the spinal cord and hypothalamus in the intervention group β-EP and ENK increased in different degrees(P<0.05);Peripheral posterior cervical muscle tissue and plasma in intervention group β-EP content increased(P<0.05),5-HT content decreased(P<0.05).Conclusion through the dynamic observation of the early,middle and l ate stages,it is found that.Early intervention
