柴胡蚤休汤对原发性肝癌患者血清MMP-9、VEGF、IGF-2水平及MSCT检查情况的影响研究

摘　　要：目的探讨柴胡蚤休汤对原发性肝癌患者血清基质金属蛋白酶-9(MMP-9)、血管内皮生长因子(VEGF)、胰岛素样生长因子-2(IGF-2)水平及多层螺旋电子计算机断层扫描(MSCT)检查情况的影响。方法选择60例原发性肝癌患者作为研究对象,按照随机数表法分为对照组与观察组,各30例。对照组予FAP化疗方案治疗,观察组在对照组基础上加用柴胡蚤休汤治疗,2组均持续治疗2个周期。比较2组治疗2个周期后临床疗效,治疗前、治疗2个周期后MSCT检查情况、肝功能、炎症反应及血清细胞因子水平。结果治疗2个周期后,观察组总有效率(66.67%,20/30)高于对照组(40.00%,12/30)(P<0.05)。与治疗前比较,治疗2个周期后,2组肿瘤数目、肝外转移占比及血清天冬氨酸氨基转移酶(AST)、丙氨酸氨基转移酶(ALT)、甲胎蛋白(AFP)、细胞缺氧诱导因子-1α(HIF-1α)、核因子κB(NF-κB)、MMP-9、VEGF、IGF-2水平降低,观察组低于对照组(P<0.05),2组肿瘤体积缩小,观察组小于对照组(P<0.05)。结论柴胡蚤休汤可降低原发性肝癌患者血清MMP-9、VEGF、IGF-2水平,减轻机体炎症反应,改善患者肝功能,控制肿瘤进展,提高临床疗效。Objective To investigate the effect of Chaihu Huaixiu Decoction on the serum levels of matrix metalloproteinase-9(MMP-9),vascular endothelial growth factor(VEGF),insulin-like growth factor-2(IGF-2)and the examination results with the multilayer spiral electron computed tomography(MSCT)in patients with primary liver cancer.Methods A total of 60 patients with primary liver cancer were selected as the research subjects and randomly divided into a control group and an observation group,with 30 cases in each group.The control group was treated with the FAP chemotherapy regimen,while the observation group was additionally treated with Chaihu Huaixiu Decoction on the basis of the control group,both for 2 cycles.The clinical efficacy,MSCT examination,liver function,inflammatory response and serum cytokine levels before treatment and after two cycles of treatment were compared between the two groups.Results After treatment,the total effective rate in the observation group(66.67%,20/30)was higher than that in the control group(40.00%,12/30)(P<0.05).Compared with before treatment,the number of tumors,the proportion of extrahepatic metastasis,and the serum levels of aspartate aminotransferase(AST),alanine aminotransferase(ALT),alpha-fetoprotein(AFP),hypoxia-inducible factor-1α(HIF-1α),nuclear factorκB(NF-κB),MMP-9,VEGF,IGF-2 in the two groups were decreased,and the observation group was lower than the control group(P<0.05).The tumor volume in the two groups was decreased as well,and the tumor volume in the observation group was smaller than that in the control group(P<0.05).Conclusion Chaihu Huaixiu Decoction can reduce the levels of serum MMP-9,VEGF and IGF-2 in patients with primary liver cancer,reduce the body’s inflammatory response,and then improve the liver function,control the tumor progression and improve the clinical efficacy.
