结肠癌患者血清CRP、CEA和CA19-9联合检测的临床意义

摘　　要：目的探讨结肠癌患者血清C反应蛋白(CRP)、癌胚抗原(CEA)、糖链抗原19-9(CA19-9)联合检测的临床意义,为临床诊断结肠癌提供依据。方法选择本院2016年1月至2020年12月收治的结肠癌患者60例作为结肠癌组,选择同期收治的结肠良性疾病患者60例作为良性病变组,另选择同期在本院进行体检的健康者50例作为对照组,比较3组血清CRP、CEA和CA19-9水平,分析血清CRP、CEA和CA19-9水平的相关性,分析血清CRP、CEA和CA19-9单独及联合检测的敏感度、特异度、准确度,并进行ROC曲线分析。结果结肠癌组血清CRP、CEA和CA19-9水平高于良性病变组和对照组,良性病变组高于对照组(P<0.05),血清CRP与CEA和CA19-9呈正相关(P<0.05),CEA与CA19-9呈正相关(P<0.05),CRP检测的敏感度69.82%、特异度64.71%、曲线下面积(AUC)为0.675,CEA检测的敏感度65.24%、特异度64.59%、AUC为0.694,CA19-9检测的敏感度63.94%、特异度68.83%、AUC为0.662,三者联合检测的敏感度93.94%、特异度88.83%、AUC为0.862,高于单项检测(P<0.05)。结论结肠癌患者血清CRP、CEA和CA19-9水平异常升高,三者联合检测可提高结肠癌的检出率,对辅助诊断结肠癌有一定意义。Objective To investigate the clinical significance of combined detection of serum C-reactive protein(CRP),carcinoembryonic antigen(CEA)and glycochain antigen 19-9(CA19-9)in patients with colon cancer,so as to provide basis for clinical diagnosis of colon cancer.Methods A total of 60 cases of colon cancer patients admitted to our hospital from January 2016 to December 2020 were selected as colon cancer group,60 cases of benign colon disease patients were selected as benign lesions group,and 50 healthy patients who underwent physical examination in our hospital during the same period were selected as the control group.Serum CRP,CEA and CA19-9 levels of the three groups were compared.The correlation of serum CRP,CEA and CA19-9 levels,and the sensitivity,specificity,accuracy,positive predictive value and negative predictive value of serum CRP,CEA and CA19-9 tests alone and in combination were analyzed,and ROC curve analysis was conducted.Results The serum levels of CRP,CEA and CA19-9 in colon cancer group were higher than those in benign lesion group and control group,while those in benign lesion group were higher than those of the control group(P<0.05).Serum CRP was positively correlated with CEA and CA19-9(P<0.05),CEA was positively correlated with CA19-9(P<0.05).The sensitivity,specificity and AUC of CRP were 69.82%,64.71%and 0.675,respectively,CEA were 65.24%,64.59%and 0.694,respectively,CA19-9 were 63.94%,68.83%and 0.662 and the sensitivity,specificity and AUC of combined detection were 93.94%,88.83%and 0.862,respectively,which were higher than those of single detection(P<0.05).Conclusion The serum levels of CRP,CEA and CA19-9 are abnormally high in patients with colon cancer.The combined detection of CRP,CEA and CA19-9 can improve the detection rate of colon cancer,which has certain significance for auxiliary diagnosis of colon cancer.
