摘　　要：[目的]探讨miR-188-5p在胃癌组织中的表达及临床意义。[方法]采用回顾性研究方法选择在本院进行诊治的120例胃癌患者(胃癌组)与120例胃良性病变(良性组)患者作为研究对象,取2组患者的病理组织,采用qRT-PCR检测miR-188-5p表达水平,调查患者的临床特征与随访患者的预后,判断miR-188-5p的诊断价值与预测预后效果。[结果]胃癌组的miR-188-5p相对表达水平与表达阳性率显著高于良性组(P<0.05)。胃癌组中不同Dukes分期、淋巴结转移、分化程度、远处转移、浸润深度患者的miR-188-5p表达阳性率对比差异有统计学意义(P<0.05)。ROC曲线分析显示miR-188-5p预测诊断胃癌的曲线下面积为0.794(95%CI:0.812~0.923),诊断阈值为1.242时,灵敏度为60.2%,特异度为95.8%。胃癌组患者随访至今,平均随访时间为(15.22±2.48)个月,死亡12例,死亡率为10.0%;单因素和多因素Cox回归分析显示miR-188-5p表达阳性率、Dukes分期、淋巴结转移、分化程度、远处转移、浸润深度均为影响胃癌患者预后的危险因素(P<0.05)。[结论]miR-188-5p在胃癌组织中呈现高表达状况,与患者的临床特征显著相关,诊断胃癌的效果比较好,可以作为胃癌发生和预后的潜在生物标志物。[Objective]To investigate the expression and clinical significance of miR-188-5 p in gastric cancer.[Methods]By using a retrospective study, 120 patients with gastric cancer(gastric cancer group)and 120 patients with benign gastric lesions(benign group)were selected as the study subjects.The expression level of miR-188-5 p in the pathological tissues of the patients were detected by qRT-PCR,and the clinical features of the patients and the prognosis of the patients were investigated and were to determine the diagnostic value and prognostic effect of miR-188-5 p.[Results]The relative expression and expression of miR-188-5 p in the gastric cancer group were significantly higher than those in the benign group(P<0.05).In the 120 patients with gastric cancer, the positive rates of miR-188-5 p expression in patients with different Dukes stage, lymph node metastasis, degree of differentiation, distant metastasis, and invasive depth were statistically significant(P<0.05).ROC curve analysis showed that the area under the curve of miR-188-5 p predictive diagnosis of gastric cancer was 0.794(95%CI 0.812-0.923).When the diagnostic threshold was 1.242,the sensitivity was 60.2% and the specificity was 95.8%.The 120 patients with gastric cancer were followed up for the average of(15.22±2.48)months, there were 12 patients were died, and the mortality rate were 10.0%.Univariate and multivariate Cox regression analysis showed miR-188-5 p expression positive rate, Dukes stage, lymph node metastasis, differentiation degree, distant metastasis, and depth of invasion were all risk factors for the prognosis of patients with gastric cancer(P<0.05).[Conclusion]miR-188-5 p are highly expressed in gastric cancer tissues, which are significantly correlated with the clinical features of patients.The diagnosis of gastric cancer is better and can be used as potential biomarkers for the occurrence and prognosis of gastric cancer.
