摘　　要：[目的]定量检测胃癌(gastric cancer, GC)患者血清microRNA;497(miR-497)表达量,探讨其与GC侵袭转移及预后的关系。[方法]以实时荧光定量RT-PCR检测70例GC患者(GC组)术前及术后、30例萎缩性胃炎患者(萎缩性胃炎组)及30例健康者(正常对照组)血清miR-497表达量,同时检测血清癌胚抗原(CEA);随访GC患者术后预后3年;分析术前血清miR-497表达量与GC临床病理特征及预后的关系。[结果]GC组术前血清miR-497表达量显著低于其术后及萎缩性胃炎组和正常对照组(P<0.01);GC组术前血清miR-497表达量与GC浸润深度、淋巴结转移、TNM分期及是否远处转移呈负相关(均P<0.01),与肿瘤分化程度、大体形态及术前血清CEA含量无相关(P>0.05)。术前血清miR-497低表达组术后2、3年生存率显著低于高表达组(P<0.05)。Cox多因素分析显示术前血清miR-497表达量是影响GC术后预后的独立高危因素。[结论]血清miR-497低表达与GC侵袭转移、不良预后密切相关,可成为GC术前及预后评价指标。[Objective]To investigate the correlation between serum miR-497 level and invasion, metastasis, prognosis of gastric cancer(GC).[Methods]The quantitative expression level of serum miR-497 was detected by real-time fluorescence quantitative RT-PCR in GC(n= 70),atrophic gastritis(n= 30)and normal control(n= 30).The serum CEA levels were also detected.The postoperative 3 years prognosis was followed-up.The relationship between the preoperative serum miR-497 expression level in the GC group and the pathologic features and prognosis were analyzed.[Results]The serum miR-497 expression level in GC group was significantly lower than those in the postoperative therapy group, atrophic gastritis group and normal control group(P<0.01).The preoperative serum miR-497 expression level in GC group was significantly negatively related with the depth of tumor invasion, the lymph node metastasis, TNM stage, the tumor distant metastasis and the postoperative survival rate(P<0.05 or 0.01).2 or 3-year survival rate of the patients with preoperative serum miR-497 low expression level was significantly lower than those in the patients with preoperative serum miR-497 high expression level.(P<0.05).Multivariate Cox regression analysis revealed that preoperative serum miR-497 expression level was independent prognostic factor for GC.[Conclusion]The preoperative low serum miR-497 expression levels are closely relative with invasion, metastasis and poor prognosis of GC,and can be a preoperative evaluation index and prognostic indicator of GC.
