摘  要:目的:研究动态增强MRI、超声扫描和钼靶在乳腺良恶性病灶诊断中的应用价值。方法:选取本院2019年1月-2021年6月收诊的乳腺占位性病变患者100例为研究对象,均经病理组织学检查确诊病灶的良恶性,均接受动态增强MRI、钼靶和彩色多普勒超声检查,收集整理三种影像学方法诊断结果与病理组织学结果。结果:动态增强MRI、钼靶、超声三种影像学方法联合应用对乳腺良恶性病灶的诊断准确率、灵敏度和特异度均高于三种影像学方法单用的诊断准确率、灵敏度、特异度(P<0.05);在三种影像学方法单用诊断时,动态增强MRI的诊断灵敏度最高,而钼靶的特异度最高,但三种方法的诊断准确率和特异度差异均无统计学意义(P>0.05);动态增强MRI的诊断灵敏度高于钼靶和超声(P<0.05);经ROC曲线分析,联合应用的AUC值最大,为0.933,其次是动态增强MRI,AUC值为0.895,再次是超声,AUC值为0.840,钼靶的AUC值最低,为0.821;一致性分析显示:联合应用与病理结果的一致性最高,Kappa值为0.943,动态增强MRI与病理结果的一致性Kappa值为0.875,钼靶和超声的Kappa值<0.7,一致性中等。结论 :动态增强MRI、超声和钼靶在乳腺良恶性病灶鉴别诊断中均有一定价值,其中动态增强MRI的灵敏度高,钼靶的特异度高,三种方法联合应用的诊断效能高于单用,临床诊断中可根据实际情况将三者结合起来应用早期鉴别诊断乳腺病灶的良恶性,指导临床治疗。Objective:To study the application value of MRI,ultrasound scanning and molybdenum target in the diagnosis of benign and malignant breast lesions.Methods:100 patients with breast placeholder lesions treated from January 2019 to June 2021 were selected as the study subjects.All lesions were benign and malignant by pathological histological examination,underwent dynamic enhanced MRI,molybdenum target and color Doppler ultrasound,and collected and sorted three imaging methods and pathological histological results.Results:The diagnostic accuracy,sensitivity and specificity of MRI imaging methods,MOR target and ultrasound were higher than the diagnostic accuracy,sensitivity and specificity of the three imaging methods(P <0.05);When the three imaging methods are diagnosed alone,Dynamic enhanced MRI has the highest diagnostic sensitivity,The molybdenum target has the highest specificity,However,the diagnostic accuracy and specificity of the three methods were not significant(P>0.05);Dynamic enhanced diagnostic sensitivity of MRI was higher than the molybdenum target and ultrasound(P<0.05);After the ROC curve analysis,Joint application AUC values are maximum,At 0.933,The second is the dynamic enhancement of the MRI,The AUC value is 0.895,The second one is the ultrasound,The AUC value is 0.840,Molmolybdenum target had the lowest AUC value,For 0.821;The consistency analysis showed that the highest agreement with the pathological results,The Kappa value was 0.943,Dynamic enhanced MRI and the pathological outcome showed a consistent Kappa value of 0.875,Kappa values <0.7 for molybdenum target and ultrasound.,The consistency was moderate.Conclusion:dynamically enhanced MRI,ultrasound and molybdenum target in breast lesions differential diagnosis,dynamic enhanced MRI sensitivity,high specificity of molybdenum target,three methods combined diagnostic efficiency is higher than single,clinical diagnosis can be combined according to the actual situation,early differential diagnosis of benign and malignant breast lesions,guide clin
