围术期配合春泽汤及针刺治疗心脏肿瘤回顾性病例系列研究

摘　　要：目的 观察原发性心脏肿瘤患者围手术期配合春泽汤及针刺治疗的临床疗效。方法 采用回顾性病例系列研究方法,分析自2018年4月—2020年11月收治的18例原发性心脏肿瘤患者病例资料,患者均在全麻体外循环下行心脏肿瘤摘取术,根据患者围术期的症状予中药汤剂春泽汤治疗,其中3例术前脑栓塞患者予针刺治疗,选穴以靳三针为主。观察患者NYHA心功能分级变化及症状、肌力变化。结果 18例患者顺利完成心房肿瘤摘取术,术后心功能均改善(P<0.05),心悸、失眠、纳差等症状明显改善,3例脑栓塞患者围术期应用针刺治疗能明显改善肢体活动能力。结论 对于原发性心脏肿瘤,外科手术安全有效。围术期配合春泽汤或针刺治疗,能够改善心悸、气短、肢体活动障碍等症状。Objective To observe the clinical effect of Chunze Decoction and acupuncture on cardiac tumor during perioperative period. Methods Retrospective case series study was used to analyze the data of 18 patients with primary cardiac tumors admitted to our hospital from April 2018 to November 2020. The patients all underwent cardiac tumor extraction under general anesthesia and cardiopulmonary bypass,and Chunze Decoction was prescribed according to the perioperative symptoms of the patients. Three patients with preoperative cerebral embolism were treated with acupuncture,and Jin’s Three-needle Therapy was the main point selection.The changes in NYHA cardiac function grade,symptoms and muscle strength were observed. Results Totally 18 patients completed atrial tumor extraction,and their cardiac function was improved(P<0.05),and symptoms such as palpitations,insomnia,and anorexia were significantly improved. The limbs activity capacity of 3 patients with cerebral embolism was significantly improved through perioperative acupuncture treatment. Conclusion For primary heart tumors,surgery is safe and effective. In the perioperative period,Chunze Decoction or acupuncture can improve patients’ symptoms such as heart palpitations,shortness of breath,limb dysfunction and so on.
