中医辨证联合三参胃炎汤治疗慢性糜烂性胃炎的临床疗效及预后的影响

摘　　要：目的:研究中医辨证联合三参胃炎汤治疗慢性糜烂性胃炎的临床疗效及预后的影响。方法:抽取青岛大学附属威海市立二院于2018年10月-2020年10月收治的慢性糜烂性胃炎患者共86例,将其依据随机抽样法分成对照组(43例,实施单纯三参胃炎汤治疗方式)和观察组(43例,实施中医辨证联合三参胃炎汤治疗方式),研究比对患者的症状评分、有效率、生活质量综合评定问卷(Generic Quality of Life Inventory-74,GQOLI-74)评分与预后情况、C反应蛋白、白细胞介素-6、糜烂面积等。结果:两组治疗前胃痛、腹胀与胃黏膜愈合等症状评分差异无统计学意义(P>0.05);与对照组相比,观察组慢性糜烂性胃炎患者治疗后的胃痛、腹胀与胃黏膜愈合等症状评分更高(P<0.05)。与对照组相比,观察组慢性糜烂性胃炎患者的总有效率更高(P<0.05)。两组治疗前GQOLI-74量表评分差异无统计学意义(P>0.05);与对照组相比,观察组慢性糜烂性胃炎患者治疗后的躯体功能、社会功能、心理功能与物质生活功能等GQOLI-74量表评分更高(P<0.05)。两组慢性糜烂性胃炎患者的局部出血、腹泻与消化不良等不良反应发生率差异无统计学意义(P>0.05)。同时观察组的白细胞介素-6、C反应蛋白和糜烂面积治疗后降低程度更显著优于对照组(P<0.05)。结论:中医辨证联合三参胃炎汤治疗对于缓解病情,迅速减轻相关症状,促进糜烂恢复等有重要意义。Objective:To study the effect and prognosis of TCM syndrome differentiation plus the Sanshen Weiyan decoction(三参胃炎汤)on chronic erosive gastritis.Methods:A total of 86 patients with chronic erosive gastritis admitted to the Second Weihai Municipal Hospital affiliated to Qingdao University from October 2018 to October 2020 were selected and divided into the control group(43 cases,treated with the Sanshen Weiyan decoction alone)and the observation group(43 cases,treated with TCM syndrome differentiation plus the Sanshen Weiyan decoction)according to random sampling method.Symptom score,response rate,generic quality of life inventory-74(GQOLI-74)score,prognosis,C-reactive protein,interleukin-6 and erosion area in two groups were compared.Results:Before treatment,there were no statistically significant differences in symptoms scores of stomachache,abdominal distension and gastric mucosa healing between two groups(P>0.05).Compared with the control group,the scores of symptoms such as stomachache,abdominal distension and gastric mucosa healing in the observation group were higher after treatment(P<0.05).Compared with the control group,the effective rate in chronic erosive gastritis patients in the observation group was higher(P<0.05).There was no significant difference in GQOLI-74 scale score between the two groups before treatment(P>0.05).Compared with the control group,the GQOLI-74 scale scores of patients with chronic erosive gastritis in the observation group were higher after treatment(P<0.05).There was no significant difference in the incidence of local bleeding,diarrhea,dyspepsia and other adverse reactions between the two groups(P>0.05).At the same time,interleukin-6,C-reactive protein and erosive area in the observation group were more significantly decreased after treatment than those in the control group(P<0.05).Conclusion:The treatment of TCM syndrome differentiation plus the Sanshen Weiyan decoction is of great significance in alleviating the condition,rapidly relieving the related symptoms and pr
