基于数据挖掘的名老中医治疗偏头痛的用药规律探析

摘　　要：目的:基于中国知识资源总库,利用中医传承计算平台V3.0探析名老中医治疗偏头痛的用药规律及特点。方法:检索中国知识资源总库1988年1月1日-2021年7月20日名老中医运用中药方剂(含针药结合)治疗偏头痛的相关文献,提取医案中的方药组成。在完成筛选后建立好数据库,运用中医传承计算平台V3.0的频数统计、聚类分析、关联规则分析等方法,统计药物使用频率、性味归经及功效分布情况,分析药物配伍规律及关联、聚类效果。结果:共筛选出91篇文献、145首方剂,涉及206味中药,其中,高频中药28味。使用频次居前3位的分别为川芎、白芍、白芷,药性以温、寒为主,药味以辛、苦为主,归经以肝、肺、脾经为主,功效以解表、补虚为主。常用药对30对,常用3味药配伍17组,聚为3类时聚类效果较好。结论:偏头痛的治疗药物以辛温寒、归肝肺脾经者为多,治疗方法以解表补虚、平肝息风、活血化瘀为主。以数据统计分析为依据考究药物的配伍对提高偏头痛的临床疗效意义重大。Objective:Based on the Chinese national knowledge infrastructure database,the medication rules and characteristics of treating migraine by famous veteran TCM practitioner were explored by the TCM inheritance calculation platform V3.0.Methods:The relevant literature in CNKI on treating migraine in TCM(including acupuncture plus medicine)from January 1,1988 to July 20,2021 were searched,and the prescription components in the medical records were extracted.After the screening,a database was established.The frequency statistics,cluster analysis,association rule analysis and other methods of TCM inheritance and calculation platform V3.0 were used to count frequency,medicinal properties,medicinal flavor,active ingredient distribution and efficacy of the relevant Chinese medicines,then the medicine compatibility rules,association rule and clustering effect were analyzed.Results:A total of 91 studies and 145 prescriptions were selected,involving in 206 Chinese medicines,including 28 high-frequency Chinese medicines.Among them,the top three are Chuanxiong(Ligusticum wallichii franch),Baishao(Paeonia alba)and Baizhi(Angelica dahurica).The medicinal properties are mainly warm and cold,the medicinal flavors are mainly pungent and bitter,the active ingredients mainly flow into the liver,lung and spleen,and the effectiveness is mainly relieving surface symptoms and strengthening body resistance.There are 30 medicine pairs of commonly used,and 17 commonly used combinations of 3 Chinese medicines.When they were grouped into 3 categories,the clustering effect was better.Conclusion:Most of Chinese medicines for migraine are pungent,warm and cold,mainly flow into the liver,lungs and spleen meridian.The main treatment methods are relieving exterior symptoms and strengthening body resistance,calming the liver and wind,promoting blood circulation and removing blood stasis.Based on the statistical analysis of data,it is of great significance to study the compatibility of Chinese medicines to improve the clinical efficacy on migraine.
