摘  要:目的采用网状Meta分析探讨硬化剂治疗颌面部静脉畸形的有效性。方法检索PubMed、Cochrane Library、Embase、Web of Science、知网、万方、维普及CBM数据库中硬化剂治疗颌面部静脉畸形的随机对照试验。采用Cochrane偏倚风险评估工具对纳入的文献进行质量评价。采用Stata 13.0软件进行网状Meta分析。结果最终纳入14篇文献,共1533例患者,9种硬化剂。网状Meta分析结果显示,无水乙醇+聚桂醇的有效率高于平阳霉素、平阳霉素+地塞米松、平阳霉素碘化油、鱼肝油酸钠、鱼肝油酸钠+平阳霉素、聚桂醇、无水乙醇、无水乙醇+平阳霉素(RR=1.96,95%CI 1.30~2.95;RR=1.85,95%CI 1.23~2.77;RR=1.77,95%CI 1.10~2.85;RR=2.22,95%CI 1.43~3.45;RR=1.90,95%CI 1.23~2.93;RR=1.70,95%CI 1.15~2.49;RR=1.61,95%CI 1.10~2.35;RR=1.61,95%CI 1.05~2.50)(均P<0.05);各药物累积排序概率曲线下面积从高到低分别为无水乙醇+聚桂醇(99.6%)>无水乙醇(72.2%)>无水乙醇+平阳霉素(68.0%)>聚桂醇(60.5%)>平阳霉素碘化油(49.7%)>平阳霉素+地塞米松(40.6%)>鱼肝油酸钠+平阳霉素(32.8%)>平阳霉素(22.3%)>鱼肝油酸钠(4.3%)。结论无水乙醇+聚桂醇治疗颌面部静脉畸形的有效率较高,可考虑在临床使用。Objective To explore the effectiveness of sclerosing agents in the treatment of maxillofacial venous malformations by means of network meta-analysis.Methods The PubMed,Cochrane Library,Embase,Web of Science,CNKI,Wanfang,VIP and CBM Database were searched for randomized controlled trials of sclerosing agents in the treatment of maxillofacial venous malformations.The Cochrane bias risk assessment tool was used to evaluate the quality of the included trials.Stata 13.0 software was used for network meta-analysis.Results Altogether 14 articles,with a total of 1533 patients and 9 sclerosing agents were included.Through network meta-analysis,it can be seen that the effective rate of absolute ethanol combined with lauromacrogol is higher than that of the other 8 drugs,namely pingyangmycin,pingyangmycin combined with dexamethasone,pingyangmycin iodized oil,sodium morrhuate,sodium morrhuate combined with pingyangmycin,lauromacrogol,absolute ethanol and absolute ethanol combined with pingyangmycin(RR=1.96,95%CI 1.30-2.95;RR=1.85,95%CI 1.23-2.77;RR=1.77,95%CI 1.10-2.85;RR=2.22,95%CI 1.43-3.45;RR=1.90,95%CI 1.23-2.93;RR=1.70,95%CI 1.15-2.49;RR=1.61,95%CI 1.10-2.35;RR=1.61,95%CI 1.05-2.50)(all P<0.05).The ranking of each drug from high to low sorted by surface under the cumulative ranking were absolute ethanol combined with lauromacrogol(99.6%)>absolute ethanol(72.2%)>absolute ethanol combined with pingyangmycin(68.0%)>lauromacrogol(60.5%)>pingyangmycin iodized oil(49.7%)>pingyangmycin combined with dexamethasone(40.6%)>sodium morrhuate combined with pingyangmycin(32.8%)>pingyangmycin(22.3%)>sodium morrhuate(4.3%).Conclusion Anhydrous ethanol combined with lauromacrogol is more effective in treating maxillofacial venous malformations,and can be considered for clinical use.
