长蛇灸治疗老年慢性心脾两虚型原发性失眠临床观察

摘　　要：目的 探讨长蛇灸治疗老年慢性心脾两虚型原发性失眠的效果。方法 将2021年2月-2022年2月江西省南昌市洪都中医院针灸门诊及内科门诊收治的90例老年慢性心脾两虚型原发性失眠患者作为研究对象,采用随机数字表法将其分为长蛇灸组、针刺组、西药组,每组30例;对比3组患者治疗前后的匹兹堡睡眠质量指数(PSQI)、下丘脑-垂体-肾上腺皮质轴(HPA轴)相关激素[促肾上腺皮质激素(ACTH)、皮质醇(COR)]水平。结果 治疗后,针刺组、西药组、长蛇灸组的PSQI评分及ACTH、COR水平均较治疗前明显降低,前后对比差异有统计学意义(P<0.05);治疗后3组之间的PSQI评分及ACTH、COR水平对比差异无统计学意义(P>0.05)。结论 采用长蛇灸治疗老年慢性心脾两虚型原发性失眠有助于改善患者的睡眠质量,纠正紊乱的激素水平。Objective To explore the effect of snake-like moxibustion on elderly chronic primary insomnia with deficiency of heart and spleen. Methods From February 2021 to February 2022,90 cases of elderly patients with primary insomnia with chronic heart and spleen deficiency were selected as the research objects. The patients were divided into snake-like moxibustion group,acupuncture group and western medicine group by random number table method,with 30 cases in each group. Pittsburgh sleep quality index(PQI)and hypothalamic-pituitary-adrenal axis(HPA axis)related hormones [ACTH and cortisol(COR)] were compared before and after treatment in 3 groups. Results After treatment,the PQI scores,ACTH and COR levels of acupuncture group,western medicine group and snake-like moxibustion group were significantly lower than those before treatment,and the difference had statistical significance(P< 0.05). After treatment,there were no significant differences in PSQI score,ACTH and COR levels among the three groups(P>0.05). Conclusion The treatment of senile chronic primary insomnia with deficiency of heart and spleen by snake-like moxibustion is helpful to improve the sleep quality and correct the disturbed hormone level.
