三维适形放疗联合热化疗治疗中晚期食管癌疗效及对患者不良反应和1年生存率的影响

摘　　要：目的研究三维适形放疗联合热化疗治疗中晚期食管癌疗效及对患者不良反应和1年生存率影响。方法选择四川大学华西广安医院诊治的中晚期食道癌患者116例随机分为单一组(n=58)和联合组(n=58),单一组使用三维适形放疗,联合组在此基础上使用热化疗,两组均以28d为一个周期,治疗2个周期。观察两组治疗后近期有效率及不良反应(恶性呕吐、放射性食管炎、骨髓抑制),并随访调查两组0.5、1年生存率。结果治疗3个月后,单一组有效率79.31%,低于联合组89.66%,差异有统计学意义(P<0.05);治疗3个月,两组恶性呕吐、放射性食管炎、骨髓抑制发生率,差异无统计学意义(P>0.05);0.5年后,两组生存率比较,差异无统计学意义(P>0.05);1年后,联合组生存率72.14%,高于单一组53.45%,差异有统计学意义(P<0.05)。结论中晚期食管癌使用三维适形放疗联合热化疗能在不增加毒副反应的情况下提高治疗有效率,且1年内生存率较好。Objective To study the efficacy of three-dimensional conformal radiotherapy combined with thermochemotherapy in the treatment of middle-advanced esophageal cancer and its effects on toxic and side effects and 1-year survival rate.Methods Totally 116 cases of patients with middle-advanced esophageal cancer who were diagnosed and treated in our hospital were randomly divided into single group(n=58)and combined group(n=58).Single group was given three-dimensional conformal radiotherapy,and combined group was given thermochemotherapy on this basis,and the two groups were treated for 2 cycles with 28 d as a cycle.The short-term effective rate and adverse reactions(malignant vomiting,radiation esophagitis,and myelosuppression)were observed in the two groups after treatment,and the half-year and 1-year survival rates of the two groups were investigated during follow up.Results After 3 months of treatment,the effective rate in single group was lower than that in combined group(79.31%vs 89.66%)(P<0.05).After 3 months of treatment,there were no significant differences in the incidence rates of malignant vomiting,radiation esophagitis and myelosuppression between the two groups(P>0.05).After half year,there was no significant difference in the survival rate between the two groups(P>0.05).After 1 year,the survival rate in combined group was higher than that in single group(72.14%vs 53.45%)(P<0.05).Conclusions Three-dimensional conformal radiotherapy combined with thermochemotherapy for middle-advanced esophageal cancer can improve the treatment effective rate without increasing the toxic and side effects,and it has good survival rate within 1 year.
