摘　　要：目的:探究在乳腺癌中PD-L1表达情况,进而探究免疫治疗在乳腺癌不同临床病理亚型中的疗效,寻找乳腺癌免疫治疗的获益人群。方法:收集筛选2011年1月至2019年6月包头市肿瘤医院诊断为原发性乳腺癌,接受新辅助化疗并经手术切除的乳腺癌存档蜡块各60例。新辅助治疗前后的蜡块分别作为对照组和观察组。应用免疫组化技术检测PD-L1,计算诊断比值比(NLR比值),并探讨其意义。结果:PD-L1蛋白在化疗前乳腺癌组织中的表达低于化疗后,差异无统计学意义(P>0.05)。PD-L1蛋白在化疗后乳腺癌组织中的表达在不同的年龄、肿瘤大小、腋窝淋巴结转移情况、临床分期、ER、PR、Her2等表达差异均无统计学意义(P>0.05)。结论:乳腺癌的PD-L1表达在化疗前后、患者年龄、肿瘤直径、淋巴结转移、临床分期、ER、PR、HER-2表达的差异中均无统计意义(P>0.05),尽管肿瘤直径>2 cm、HER-2(-)者PD-L1阳性表达率高,但结果显示无统计意义。Objective:Explore the expression of PD-L1 in different clinicopathological subtypes of breast cancer,and then explore the efficacy of immunotherapy in different clinicopathological subtypes of breast cancer.Looking for the benefit of breast cancer immunotherapy.Methods:To collect and screen 60 cases of breast cancer archive wax blocks diagnosed as primary breast cancer in Baotou Tumor Hospital from January 2011 to June 2019,received neoadjuvant chemotherapy and surgically removed.The wax blocks before and after neoadjuvant treatment were used as the control group and the experimental group.Use immunohistochemical technology to detect PD-L1,calculate NLR value,and discuss its significance.Use SPSS24.0 statistical software for statistical analysis,using chi-square test and independent sample t test,P<0.05 indicates that the difference is statistically significant.Results:The expression of PD-L1 protein in breast cancer tissues before chemotherapy was lower than that after chemotherapy,and the difference was not statistically significant(P>0.05).There was no significant difference in the expression of PD-L1 protein in breast cancer tissue after chemotherapy with age,tumor size,axillary lymph node metastasis,clinical stage,ER,PR,and Her2 expression(P>0.05).Conclusion:The expression of PD-L1 in breast cancer was not statistically significant in the differences in the expression of PD-L1 before and after chemotherapy,patient age,tumor diameter,lymph node metastasis,clinical stage,ER,PR,and HER-2 expression(P>0.05),although the tumor diameter>2cm,The positive expression rate of PD-L1 in HER-2(-)patients is high,but the results show no statistical significance.
