摘  要:目的观察3D-Slicer软件辅助定位下微创穿刺引流术治疗基底节区脑出血患者的临床效果。方法61例基底节区脑出血患者随机分为两组:观察组31例,根据CT信息在3D-Slicer软件辅助下行微创穿刺引流术治疗;对照组30例,仅根据CT信息简易定位下行微创穿刺引流术治疗。记录两组手术时间、住院时间、术后12 h和3 d时脑内血肿清除率、术后3 d时再出血发生率;随访30 d,观察两组病死率、改良Rankin量表(mRS)评分及临床治疗效果。结果与对照组相比,观察组手术时间缩短,术后脑内血肿清除率及mRS评分<3分患者比例增加(P<0.05或P<0.01)。观察组术后再出血发生率、住院时间和病死率稍少于对照组(P>0.05)。观察组治疗总有效率高于对照组(96.77%vs.73.33%)(P<0.05)。结论3D-Slicer软件辅助定位微创穿刺引流术治疗基底节区脑出血患者的临床效果显著,可作为一种有效的术前精准定位辅助工具。Objective To observe the clinical effect of 3 D-Slicer software assisted localization in minimally invasive puncture and drainage in the treatment of patients with cerebral hemorrhage at basal ganglia.Methods A total of 61 patients with cerebral hemorrhage at basal ganglia were randomly divided into two groups of A(31 cases)and B(30 cases).The patients in group A accepted 3 D-Slicer software-assisted localization according to the CT information in minimally invasive puncture and drainage,while those in group B only received simple localization according to the CT information in minimally invasive puncture and drainage.The operation time,hospital stay,clearance rate of intracerebral hematoma at the 12;hour and 3;day after operation,and incidence of rebleeding in the 3;day after operation were recorded in both groups.All the patients were followed up for 30 days,and the mortality,modified Rankin scale(mRS)score and clinical outcomes were observed.ResultsCompared with group B,the operation time was decreased,and the clearance rate of intracerebral hematoma and proportion of mRS score<3 points after operation were increased in group A(P<0.05 or P<0.01).The incidence of rebleeding,hospital stay and mortality in group A were slightly less than those in group B(P>0.05).The total effectiveness rate in group A was higher than that in group B(96.77%vs.73.33%)(P<0.05).Conclusion The 3 D-Slicer software-assisted localization in minimally invasive puncture and drainage has a remarkable clinical efficacy in the treatment of patients with cerebral hemorrhage at basal ganglia,and could be used as an effective auxiliary tool for the precise localization before operation.
