腹腔镜术后应用戈舍瑞林缓释植入剂对子宫内膜异位症患者性激素及sICAM-1、MCP-1、HE4水平的影响

摘　　要：目的:探究腹腔镜术后应用戈舍瑞林缓释植入剂对子宫内膜异位症(EMS)患者性激素及可溶性细胞间黏附分子1(sICAM-1)、单核细胞趋化蛋白1(MCP-1)、人附睾蛋白4(HE4)水平的影响。方法:选取我院2019年6月至2020年6月期间98例EMS行腹腔镜术后患者作为研究对象,依据治疗药物不同分为口服组和注射组。口服组49例给予地屈孕酮片口服治疗,注射组49例给予醋酸戈舍瑞林缓释植入剂皮下注射治疗,对比两组患者临床症状改善情况、性激素、血清相关因子以及术后12个月内复发情况。结果:注射组治疗6月末痛经、月经异常、性交痛水平优于口服组(P<0.05)。两组治疗6月末E_(2)、FSH、LH水平较治疗前降低,注射组低于口服组(P<0.05)。两组治疗6月末sICAM-1、MCP-1、HE4水平较治疗前降低,注射组低于口服组(P<0.05)。注射组术后12个月内复发率2.04%(1/49)低于口服组6.12%(3/49),无差异(P>0.05)。结论:腹腔镜术后应用戈舍瑞林缓释植入剂治疗EMS患者,通过改善卵巢功能紊乱状态,调控性激素水平,改善痛经、性交痛等症状,调节血清相关因子水平,降低EMS复发,提高临床疗效。Objective:To investigate the effects of goserelin sustained-release implant on the levels of sex hormone,soluble intercellular adhesion molecule-1(sICAM-1),monocyte chemoattractant protein-1(MCP-1)and human epididymal protein-4(HE4)in patients with endometriosis(EMS).Methods:A total of 98 patients who underwent laparoscopic EMS surgery in our hospital from June 2019 to June 2020 were selected as the research objects,and were divided into oral group and injection group according to different treatment drugs.In the oral group,49 patients were treated with dydrogesterone tablets orally,and in the injection group,49 patients were treated with goserillin acetate sustained-release implant subcutaneously.The improvement of clinical symptoms,sex hormones,serum related factors and recurrence within 12 months after surgery were compared between the two groups.Results:The levels of dysmenorrhea,abnormal menstruation and painful intercourse in the injection group were better than those in the oral group(P<0.05).At the end of 6 months after treatment,the levels of E2,FSH and LH in the injection group were lower than those in the oral group(P<0.05).At the end of 6 months after treatment,the levels of sICAM-1,MCP-1 and HE4 in the injection group were lower than those in the oral group(P<0.05).The recurrence rate within 12 months in the injection group was 2.04%(1/49)lower than 6.12%(3/49)in the oral group(P>0.05).Conclusion:After laparoscopic surgery,goseralin sustained release implant was applied to treat EMS patients,which can improve the ovarian dysfunction,regulate the level of sex hormones,improve dysmenorrhea,pain during intercourse and other symptoms,regulate the level of serum related factors,reduce the recurrence of EMS and improve the clinical efficacy.
