摘　　要：目的观察口服2.5%甘露醇溶液后小肠超声造影检查(SICUS)诊断小肠克罗恩病(CD)及其并发症的价值。方法前瞻性纳入135例小肠CD患者,根据C反应蛋白(CRP)水平将其分为轻微组(n=66)和严重组(n=69),对比观察常规肠道超声(IUS)、SICUS及MR小肠造影(MRE)诊断小肠CD及其并发症的效能。结果对所有135例及轻微组66例患者,SICUS对检出小肠CD的敏感度均高于IUS而低于MRE(P均<0.05);对严重组69例患者,SICUS检出小肠CD的敏感度高于IUS(P<0.05),而与MRE差异无统计学意义(P>0.05)。所有患者中,SICUS检出小肠CD的特异度高于MRE(P<0.05)。SICUS检出小肠CD合并狭窄的敏感度均高于IUS及MRE(P均<0.05),而其特异度差异均无统计学意义(P均>0.05)。结论口服2.5%甘露醇溶液后SICUS有助于诊断小肠CD及其并发症,特别是肠腔狭窄;结合MRE可提高检出轻微小肠CD的敏感度。Objective To observe the value of oral administration of 2.5%mannitol solution for small intestine contrast ultrasonography(SICUS)in diagnosis of small bowel Crohn disease(CD)and relative complications.Methods A total of 135 patients with small bowel CD were prospectively enrolled and divided into mild group(n=66)and severe group(n=69)according to the level of C reactive protein(CRP).The efficacy of conventional intestinal ultrasound(IUS),SICUS and MR enterography(MRE)for diagnosing small bowel CD and relative complications were observed and compared.Results For all 135 patients and 66 patients in mild group,the sensitivities of SICUS for detecting small bowel CD were higher than those of IUS but lower than those of MRE(all P<0.05),while for 69 patients in severe group,the sensitivity of SICUS for detecting small bowel CD was higher than that of IUS(P<0.05)but not significant different with that of MRE(P>0.05).Among all patients,the specificity of SICUS for detecting small bowel CD was higher than that of MRE(P<0.05).The sensitivity of SICUS for detecting small bowel CD complicated with stenosis was higher than that of IUS and MRE(both P<0.05),but the specificities were not significantly different(both P>0.05).Conclusion Oral administration of 2.5%mannitol solution for SICUS was helpful for diagnosing small bowel CD and relative complications,especially intestinal stenosis.Combining with MRE could improve the sensitivity for detecting mild CD of small bowel.
