摘  要:目的探讨低频脉冲电刺激联合前庭功能训练对后循环脑梗死患者眩晕和平衡功能的影响。方法选取2017年1月至2020年10月浙江省台州医院神经内科就诊治疗的后循环脑梗死患者72例,采用随机数字表法将患者分为对照组(n=36)与观察组(n=36)。对照组予常规前庭功能训练,观察组在对照组基础上加低频脉冲电刺激治疗,两组均治疗8周。观察并比较两组治疗前与治疗8周后眩晕、肢体平衡情况及椎动脉超声的变化。结果治疗8周后,对照组和观察组眩晕评定量表评分较治疗前明显下降,BBS评分较治疗前明显上升,差异有统计学意义(P<0.05或P<0.01),观察组眩晕评定量表评分低于对照组、BBS评分高于对照组,差异有统计学意义(P<0.05);同时,对照组和观察组椎动脉内径和平均血流较治疗前明显上升,椎动脉阻力指数较治疗前明显下降,差异有统计学意义(P<0.05或P<0.01),观察组椎动脉内径与平均血流均高于对照组、阻力指数低于对照组,差异有统计学意义(P<0.05)。结论低频脉冲电刺激联合前庭功能训练治疗后循环脑梗死患者可减轻眩晕,改善平衡功能,其作用机制与其可扩张椎动脉内径、提高脑血流、降低脑血管阻力密切相关。Objective To explore the effect of low-frequency pulse electrical stimulation combined with vestibular function training on vertigo and balance function in patients with posterior circulation cerebral infarction.Methods A total of 72 patients with posterior circulatory cerebral infarction treated in the Department of Neurology of Taizhou Hospital of Zhejiang Province affiliated to Wenzhou Medical University from January 2017 to October 2020 were selected.They were divided into the control group(n=36)and the observation group(n=36)by random number table method.The control group was given conventional vestibular function training,and the observation group was given low-frequency pulse electrical stimulation on the basis of the control group.Both groups were treated for 8 weeks.The vertigo,limb balance and ultrasound changes of vertebral artery were observed and compared between the two groups before the treatment and after 8 weeks of treatment.Results After 8 weeks of treatment,the scores of vertigo rating scale in the control group and the observation group were significantly lower than those before treatment,and the BBS score was significantly higher than that before treatment,and there was significant difference in both groups between before and after treatment(P<0.05 or P<0.01).After treatment,the change of the observation group was more obvious than that of the control group,and there was significant difference between the two groups after treatment(P<0.05).At the same time,the internal diameter and mean blood flow of vertebral artery in the control group and the observation group were significantly increased after the treatment,and the vertebral artery resistance index was significantly lower than that before the treatment,and there was significant difference before and after treatment(P<0.05 or P<0.01).After treatment,the change of the observation group was more obvious than that of the control group,and there was significant difference between the two groups after treatment(P<0.05).Conclusion Low-frequency
