摘 要:目的探讨高频超声在玻璃体病变诊断中的临床应用价值。方法选取2012年1月至2016年3月在沈阳市第四人民医院确诊的70例玻璃体病变患者作为研究对象并进行回顾性研究,利用常规超声检查观察玻璃体病变的位置、内部回声、形态、边界、运动及后运动情况,利用彩色多普勒超声和频谱多普勒超声检查观察、判断玻璃体病变内部和边缘的血供情况,将高频超声检查的结果和手术结果进行对比分析。结果高频超声表现,视网膜脱离:玻璃体内条状或类似“V”形的条带状回声,其上可见与视网膜中央动、静脉相延续的彩色血流信号;球内异物:玻璃体内强回声;视网膜下出血:眼球后极部扁平性隆起病变,病变表面可探及彩色血流信号,病变内部无彩色血流信号;晶状体脱位:玻璃体内部不同位置的类似椭圆形异常回声,周边部呈强回声,内部呈无回声区,高频超声检查结果与手术结果相符合的例数为视网膜脱离25例,球内异物15例,视网膜下出血19例,晶状体脱位7例,正确率为94.3%(66/70)。结论高频超声对玻璃体病变的诊断正确率较高,可为眼科临床诊断和治疗提供可靠的影像学依据。Objective To explore the clinical application value of high-frequency ultrasound in the diagnosis of vitreous lesions.Methods A total of 70 patients with vitreous lesions diagnosed in the Fourth People’s Hospital of Shenyang from January 2012 to March 2016 were selected as the research objects and retrospectively studied.The location,internal echo,morphology,boundary,motion,and post-motion condition of the vitreous lesions were observed by conventional ultrasound.The blood supply inside and around the vitreous lesion was observed and judged using color Doppler ultrasound and spectral Doppler ultrasound.The results of high-frequency ultrasound and surgery were compared and analyzed.Results High-frequency ultrasound showed retinal detachment had a stripe-like or"V"-like an echo in the vitreous,on which a color blood flow signal that continued with the central artery and vein of the retina could be seen;foreign body in the bulb had strong echo inside the vitreous.High-frequency ultrasound of subretinal hemorrhage showed flat bulging lesions on the posterior pole of the eye.Colored blood flow signals could be detected on the surface of the lesions,and no color blood flow signals could be seen inside the lesions.High-frequency ultrasound of lens dislocation showed abnormal elliptical echoes at different positions inside the vitreous body,with strong echoes in the periphery and anechoic areas in the interior.The number of cases in which the results of high-frequency ultrasonography were consistent with the surgical results was:25 cases of retinal detachment,15 cases of intra-ball foreign body,19 cases of subretinal hemorrhage,and 7 cases of lens dislocation.The correct rate was 94.3%(66/70).Conclusion High-frequency ultrasound has a high accuracy rate in the diagnosis of vitreous lesions,which can provide reliable imaging evidence for clinical diagnosis and treatment of ophthalmology.
