CYFRA21-1、CA19-9、SCCAg在食管癌诊断中的应用价值

摘　　要：目的探究细胞角质蛋白19片段(CYFRA21-1)、糖类抗原19-9(CA19-9)、鳞状细胞癌抗原(SCCAg)在食管癌诊断中的应用价值。方法选择食管癌患者102例为研究组,另选取同时间体检的54例健康志愿者作为对照组。比较2组CYFRA21-1、CA19-9、SCCAg水平及不同分期食管癌患者CYFRA21-1、CA19-9、SCCAg水平。以病理学结果为“金标准”,分析CYFRA21-1、CA19-9、SCCAg单独及联合检测在食管癌诊断中的价值,另分析CYFRA21-1、CA19-9、SCCAg单独及联合检测与病理学结果的一致性。结果研究组CYFRA21-1、CA19-9、SCCAg水平均高于对照组,有统计学差异(P<0.05)。食管癌Ⅲ~Ⅳ期患者CYFRA21-1、CA19-9、SCCAg水平高于Ⅰ~Ⅱ期患者,有统计学差异(P<0.05)。CYFRA21-1、CA19-9、SCCAg联合检测诊断食管癌的灵敏度、特异度、准确度均高于单项检测,有统计学差异(P<0.05);kappa检验显示:CYFRA21-1与病理学结果的一致性尚可(kappa值=0.409,P=0.000);CA19-9与病理学结果的一致性不佳(kappa值=0.329,P=0.000);SCCAg与病理学结果的一致性不佳(kappa值=0.201,P=0.005);联合检测与病理学结果的一致性尚可(kappa值=0.706,P=0.000)。结论CYFRA21-1、CA19-9、SCCAg联合检测在食管癌诊断中应用价值较高,依据其水平变化能够及时判断病情严重程度,且与病理学检查结果一致性较强,可为临床诊断提供参考依据,值得推广。Objective To investigate the application value of cell keratin 19 fragment(CYFRA21-1),sugar antigen 19-9(CA19-9),and squamous cell cancer antigen(SCCAg)in the diagnosis of esophageal cancer.Methods 102 patients with esophageal cancer were selected as the study group,and 54 health volunteers were selected as the control group.The CYFRA21-1,CA19-9,SCCAg levels of the 2 groups and the CYFRA21-1,CA19-9,SCCAg levels were compared in different stages;the diagnostic value of CYFRA21-1,CA19-9,SCCAg alone and jointly in esophageal cancer was analyzed,and the consistency of CYFRA21-1,CA19-9,SCCAg alone and pathological results was analyzed.Results The CYFRA21-1,CA19-9,SCCAg level in the study group were higher than the control group,There were statistical differences(P<0.05);Patients with stageⅢ~Ⅳesophageal cancer had higher CYFRA21-1,CA19-9,SCCAg than stageⅠ~Ⅱesophageal cancer patients,There were statistical differences(P<0.05);CYFRA21-1,CA19-9,SCCAg combined testing had higher sensitivity,specificity and accuracy in the diagnosis of esophageal cancer than single testing,there were statistical differences(P<0.05);kappa test showed acceptable consistency of CYFRA21-1 with pathological results(kappa=0.409,P=0.000);CA19-9 was not consistent with pathological results(kappa=0.329,P=0.000);SCCAg was not consistent with pathological results(kappa=0.201,P=0.005);Joint testing and pathological results were consistent(kappa=0.706,P=0.000).Conclusion CYFRA21-1,CA19-9,SCCAg combined test has high value in esophageal cancer diagnosis,timely determines the severity of the disease and has strong consistency with pathological examination,which can provide a reference for clinical diagnosis and is worth popularizing.
