摘　　要：目的获得昆明市16岁及以上男性发生非婚异性性行为比例及其影响因素,为男性艾滋病高风险人群规模评估和干预提供参考数据。方法采用横断面调查,使用多阶段抽样方法在昆明市5个县区开展16岁及以上男性街头拦截问卷调查,使用χ^(2)检验和Logistic回归分析影响因素。结果有效问卷共10283份,82.8%(8517/10283)的调查对象发生过异性性行为,其中,最近一年发生过商业性性行为的比例为4.7%(486/10283)、最近一年发生过非婚非商业性性行为的比例为7.4%(764/10283)、最近一年同时发生过以上两种行为的比例为3.7%(381/10283)。发生商业性性行为的相关因素包括大专以下学历、招募地点为休闲娱乐场所;发生非婚非商业性性行为的相关因素包括年龄50岁及以下、文盲、离异或丧偶者、招募地点为休闲娱乐场所;以上两种行为均有发生的相关因素包括:年龄(16~20岁、21~30岁、41~50岁)、学历(文盲、初中、高中或中专)、调查地区类别为城区。结论16岁及以上男性发生非婚异性性行为比例较高,应加大推广性健康教育的力度,重点关注低学历、性活跃年龄段人群、长期处于无配偶陪伴状态下的男性群体,同时,有效开展城区娱乐场所的宣教工作。Objective To obtain the proportion of non-marital heterosexual behavior among males over 15 years old in Kunming and its influencing factors,aiming to provide reference data for the size estimation and intervention of males with high risk.Methods A multi-stage sampling method was used to carry out a questionnaire survey among men over15 years old in 5 counties and districts of Kunming City.The influencing factors were analyzed byχ^(2)tests and logistic regression.Results A total of 10,283 valid questionnaires were received with 82.8%(8517/10283)of the respondents having heterosexual sexual behavior,4.7%(486/10283)having commercial sexual behavior in the past year.The proportion of married non-commercial sexual behavior was 7.4%(764/10283),and the proportion of both commercial and non-marital non-commercial sexual behavior in the last year was 3.7%(381/10283).The risk factors for commercial sexual behavior included:having a college degree or less and being recruited from leisure and entertainment locations.The risk factors for non-marital non-commercial sexual behavior included:aged 50 or less,being illiterate,being divorced or widowed,and being recruited from leisure and entertainment places.Risk factors for both commercial and non-marital non-commercial sexual behavior included:being 16-20 years old,being 21-30 years old,being 41-50 years old,Beijing illiterate,with the education of junior high school,high school,or technical secondary school,and located in urban areas.Conclusion The proportion of non-marital heterosexual behavior among adult men over 15 years of age was relatively high.The promotion of sexual health education should be intensified.We need to focus on low-literate,sexually active age groups,and people with long-term unaccompanied.Publicity and education in key areas such as urban entertainment venues should be carried out effectively.
