微小RNA-542-3p和Toll样受体4在癫痫患者血浆中的表达及临床意义

摘　　要：目的观察微小RNA-542-3p(miR-542-3p)、Toll样受体4(TLIR4)在癫痫患者血浆中的表达水平,并讨论其临床意义。方法选取2014年10月至2019年10月于长江大学附属仙桃市第一人民医院神经内、外科收治的原发性癫痫患者134例作为观察组,另选取同时期于长江大学附属仙桃市第一人民医院体检结果证实健康者140例作为对照组。实时荧光定量PCR(qRT-PCR)测定受试者血浆中miR-542-3p的表达水平;应用酶联免疫吸附实验(ELASA)测定两组受试者血浆中TLR4的表达水平。受试者工作特征(ROC)曲线分析癫痫患者血浆中miR-542-3p、TLR4对癫痫不同发作类型的预测价值。Pearson分析miR-542-3p与TLR4的相关性。结果观察组血浆中miR-542-3p表达水平低于对照组(P<0.05),TLR4表达水平高于对照组(P<0.05);全面性发作患者血浆miR-542-3p表达水平显著低于部分性发作患者,TLR4表达水平高于部分性发作患者(P<0.05);血浆miR-542-3p对癫痫不同发作类型预测的ROC曲线下面积为0.886,截断值为0.838,敏感度为87.3%,特异性为76.2%;TLR4对癫痫不同发作类型预测的ROC曲线下面积为0.803,截断值为2.285 ng·mL^(-1),敏感度为83.1%,特异性为71.4%;两者联合检测对癫痫不同发作类型预测的ROC曲线下面积为0.926,敏感度为93.0%,特异性为85.7%;癫痫患者血浆中miR-542-3p与TLR4呈负相关(r=-0.610,P<0.05)。结论癫痫患者血浆中miR-542-3p表达水平下降,而TLR4表达升高,两者之间负相关,未来或可作为预测癫痫发作类型的有效指标。Objective To observe the expression levels of microRNA-542-3p(miR-542-3p) and toll like receptor 4(TLR4) in the plasma of epileptic patients and discuss their clinical significance.Methods From October 2014 to October 2019,134 primary epileptic patients in Neurology and Neurosurgery of Xiantao first people’s Hospital Affiliated to Changjiang University were selected as the observation group,and 140 healthy subjects examined in Xiantao first people’s Hospital Affiliated to Changjiang University were selected as the control group.The expression level of miR-542-3p in plasma was measured by real-time fluorescence quantitative PCR(qRT-PCR);enzyme linked immunosorbent assay(ELASA) was used to detect the expression of TLR4 in the plasma of the two groups.The receiver operating characteristic(ROC) curve was used to analyze the predictive values of miR-542-3p and TLR4 in epileptic patients’ plasma for different seizure types.Pearson was used to analyze the correlation between miR-542-3p and TLR4.Results The expression level of miR-542-3p in the plasma of the observation group was lower than that of the control group(P <0.05),and the expression level of TLR4 was higher than that of the control group(P <0.05);the expression level of plasma miR-542-3p in patients with comprehensive attack was significantly lower than that in patients with partial attack,and the expression level of TLR4 was higher than that in patients with partial attack(P <0.05);the area under ROC curve of plasma miR-542-3p was 0.886,the cutoff value was 0.838,the sensitivity was 87.3%,and the specificity was 76.2%;the area under ROC curve of TLR4 predicting different seizure type was 0.803,the cutoff value was 2.285 ng·mL^(-1),the sensitivity was 83.1%,and the specificity was 71.4%;the area under ROC curve of their combination predicting different seizure type was 0.926,the sensitivity was 93.0%,and the specificity was 85.7%;there was a negative correlation between miR-542-3p and TLR4(r=-0.610,P <0.05).Conclusions The expression level of miR-542-3p
