摘　　要：目的分析同源框基因A10(HOXA10)与肝素结合表皮生长因子(HB-EGF)在子宫内膜异位症(EMs)合并不孕患者中的表达及意义。方法选取2016年1月至2019年12月西安高新医院妇产科收治的EMs及不孕患者共170例,根据患病类型将其分为EMs合并不孕组(70例)和单纯EMs组(100例),另选取同期行手术治疗的60例子宫肌瘤患者作为对照组。所有患者行血清及子宫内膜HOXA10、HB-EGF测定。比较三组患者的血清HOXA10、HB-EGF水平,分析血清HOXA10、HB-EGF与EMs合并不孕患者r-AFS分期的相关性,比较不同r-AFS分期EMs合并不孕患者的血清HOXA10、HB-EGF水平,比较不同r-AFS分期EMs及对照组患者的子宫内膜HOXA10、HB-EGF表达情况,比较EMs合并不孕组与单纯EMs组患者的子宫内膜HOXA10、HB-EGF表达情况。结果对照组的血清HOXA10、HB-EGF水平显著高于EMs合并不孕组和单纯EMs组,差异具有统计学意义(P<0.05);单纯EMs组的血清HOXA10、HB-EGF水平显著高于EMs合并不孕组,差异具有统计学意义(P<0.05)。Pearson相关性分析结果显示,血清HOXA10、HB-EGF与EMs合并不孕患者r-AFS分期呈负相关(r=-0.324、-0.313,P<0.05)。Ⅰ~Ⅱ期EMs合并不孕患者的血清HOXA10、HB-EGF水平显著高于Ⅲ~Ⅳ期EMs合并不孕患者,差异具有统计学意义(P<0.05)。对照组的子宫内膜HOXA10、HB-EGF表达水平显著高于Ⅰ~Ⅱ、Ⅲ~Ⅳ期EMs,差异具有统计学意义(P<0.05);Ⅰ~Ⅱ期EMs的子宫内膜HOXA10、HB-EGF表达水平显著高于Ⅲ~Ⅳ期EMs,差异具有统计学意义(P<0.05)。单纯EMs组的子宫内膜HOXA10、HB-EGF表达水平高于EMs合并不孕组,差异具有统计学意义(P<0.05)。结论在EMs合并不孕患者中,HOXA10、HB-EGF水平显著降低,二者均参与了该疾病的发生、发展,有望成为诊断和监测该疾病的可靠指标。Objective To analyze the expression and significance of homeobox A10(HOXA10)and heparin-binding epidermal growth factor(HB-EGF)in patients with endometriosis(EMs)complicated with infertility.Methods A total of 170 patients with EMs and infertility admitted in the gynecology and obstetrics department of Xi'an Gaoxin hospital from January 2016 to December 2019 were selected and divided into EMs complicated with infertility group(70 cases)and simple EMs group(100 cases)according to the type of disease,the another 60 patients with hysteromyoma who underwent surgical treatment at the same period were selected as control group.HOXA10 and HB-EGF in serum and endometrium were measured in all patients.The serum HOXA10 and HB-EGF levels among the three groups were compared,the correlation between serum HOXA10,HB-EGF and r-AFS stage of patients with EMs complicated infertility was analyzed,the serum HOXA10 and HB-EGF levels in patients with EMs complicated infertility at different r-AFS stages were compared,the expressions of endometrium HOXA10 and HB-EGF in patients with EMS at different r-AFS stages and the control group were compared,and the expressions of endometrium HOXA10 and HB-EGF in patients of the EMs complicated infertility group and the simple EMs group were compared.Results The serum HOXA10 and HB-EGF levels in the control group were significantly higher than those in the EMs complicated infertility group and the simple EMs group,and the differences were statistically significant(P<0.05);the serum HOXA10 and HB-EGF levels in the simple EMs group were significantly higher than those in the EMs complicated infertility group,and the differences were statistically significant(P<0.05).Pearson correlation analysis result showed that serum HOXA10 and HB-EGF were negatively correlated with r-AFS stage of patients with EMs complicated infertility(r=-0.324,-0.313,P<0.05).The serum HOXA10 and HB-EGF levels in patients with stageⅠtoⅡEMs complicated with infertility were significantly higher than those in patients with
