摘　　要：韦格纳肉芽肿(wegener’s granulomatosis,WG)又称肉芽肿伴多发性血管炎(granulomatosis with polyangiitis,GPA),是一种极为罕见的长期系统性疾病,涉及肉芽肿的形成和血管的炎症。WG可影响多个器官中的中小血管,但最常见的是影响上呼吸道、肺、肾脏和鼻腔。典型的症状和体征包括流鼻血、鼻塞和鼻分泌物结痂,以及眼睛的葡萄膜层发炎。鼻腔鼻窦常早期受累,症状不典型,诊断困难。文章对近年来关于鼻腔鼻窦韦格纳肉芽肿的临床及影像学表现进行了文献复习,从临床表现、影像学特征、实验室检查和组织学诊断阐述鼻腔鼻窦韦格纳肉芽肿的诊断,以期为临床工作提供一定的参考。Wegener’s granulomatosis(WG),also known as granulomatosis with polyangiitis(GPA),is an extremely rare long-term systemic disease,involving the formation of granuloma and vascular inflammation.WG can affect small and medium blood vessels in multiple organs,but the most common is to affect the upper respiratory tract,lung,kidney and nasal cavity.Typical symptoms and signs include nosebleed,nasal congestion,crusting of nasal secretions,and inflammation of the uveal layer of the eye.Nasal cavity and paranasal sinus are often involved in the early stage,with atypical symptoms and difficult diagnosis.This paper reviews the literature on the clinical and imaging manifestations of Wegener’s granuloma in nasal cavity and paranasal sinus in recent years,and expounds the diagnosis of Wegener’s granuloma in nasal cavity and paranasal sinus from clinical manifestations,imaging characteristics,laboratory examination and histological diagnosis,in order to provide some reference for clinical work.
