摘　　要：目的研究封闭式负压引流(VSD)在食管癌术后吻合口瘘伤口引流中的效果,为临床治疗提供参考依据。方法选择2018年1月至2020年12月在中山市人民医院住院的60例食管癌术后吻合口瘘患者为研究对象,应用随机数字表法将患者分为研究组(n=30)和对照组(n=30)。对照组采用常规吻合口瘘引流及伤口氧疗,研究组在对照组的基础上采用VSD进行瘘口引流。比较两组的瘘口渗液量、瘘口愈合时间、住院时间、住院费用及患者生活质量。结果研究组的瘘口渗液量少于对照组,差异有统计学意义(P<0.05)。研究组的瘘口愈合时间、住院时间短于对照组,住院费用少于对照组,差异有统计学意义(P<0.05)。干预后,研究组患者的生活质量高于对照组,差异有统计学意义(P<0.05)。结论VSD是食管癌术后吻合口瘘伤口引流的有效方法,能减少瘘口渗液量,促进伤口愈合,缩短住院时间,降低住院费用,有效改善患者生活质量。Objective To study the effect of vacuum sealing drainage(VSD)in wound drainage of patients with anastomotic fistula after esophageal cancer surgery,and to provide reference basis for clinical therapy.Methods From January 2018 to December 2020,60 patients with postoperative anastomotic fistula of esophageal cancer hospitalized in Zhongshan People's Hospital were selected as the research objects.The patients were divided into the study group(n=30)and the control group(n=30)according to random number table method.The control group was treated with conventional anastomotic fistula drainage and wound oxygen therapy,and the study group was treated with VSD for fistula drainage on the basis of the control group.The leakage volume of fistula,fistula healing time,hospitalization time,hospitalization expenses and quality of life were compared between the two groups.Results The leakage volume of fistula of the study group was less than that of the control group,the difference was statistically significant(P<0.05).The fistula healing time and hospitalization time of the study group were shorter than those of the control group,and the hospitalization expenses of the study group was less than that of the control group,the differences were statistically significant(P<0.05).After the intervention,the quality of life of the study group was higher than that of the control group,the difference was statistically significant(P<0.05).Conclusion VSD is an effective method for postoperative anastomotic fistula wound drainage of esophageal cancer.It can reduce the amount of leakage,promote wound healing,shorten hospitalization time,reduce hospitalization expenses,and effectively improve the quality of life of patients.
