放射性粒子植入对转移性胸腺瘤伴重症肌无力患者相关抗体表达及淋巴细胞亚群变化的影响

摘　　要：目的研究放射性粒子植入治疗对转移性胸腺瘤伴重症肌无力(MG)患者抗体表达及淋巴细胞亚群变化。方法抽取2017年10月至2018年12月收治的转移性胸腺瘤伴MG患者共120例,将其遵照随机抽样法分成对照组[60例,施行三维适形调强放疗(3DIMRT)方式]和观察组(60例,施行放射性粒子植入治疗方式),研究比对患者的总缓解率、不良反应,以及不同治疗节点的肿瘤病灶大小、抗体表达情况与T淋巴细胞亚群变化等。结果观察组总缓解率(81.67%)明显高于对照组总缓解率(53.33%),差异有统计学意义(P<0.05)。治疗60 d、90 d后,观察组AChR-Ab指标均明显低于对照组(P<0.05)。治疗60 d、90 d后,观察组CD_(4)^(+)/CD_(8)^(+)与CD_(4)^(+)指标均明显低于对照组,而CD_(8)^(+)指标明显高于对照组(P<0.05)。治疗60 d、90 d后,观察组肿瘤病灶直径均明显小于对照组(P<0.05)。观察组不良反应发生率(8.33%)明显低于对照组不良反应发生率(21.67%),差异有统计学意义(P<0.05)。结论放射性粒子植入治疗对于缓解病情,提高临床疗效,改善抗体表达等有重要意义。Objective To investigate the effects of radioactive particle implantation on the expression of related antibodies and lymphocyte subsets in patients with metastatic thymoma complicated by myasthenia gravis(MG).Methods A total of 120 patients with metastatic thymoma complicated by MG who were treated in our hospital from October 2017 to December 2018 were enrolled in the study,who were randomly divided into control group and observation group,with 60 cases in each group.The patients in control group were treated by 3DIMRT,however,thepatientsin observation group were treated by radioactive particle implantation.The total response rate,adverse reactions,tumor lesion size,antibody expression and T lymphocyte subsets were observed and compared between the two groups.Results The total remission rate in observation group was 81.67%,which was significantly higher than that(53.33%)in control group(P<0.05).After 60-day and 90-day treatment,the levels of AChR-Ab and CD_(4)^(+)/CD_(8)^(+) and CD_(4)^(+) in observation group were significantly lower than those in control group(P<0.05),however,the levels of CD_(8)^(+) were significantly higher than those in control group(P<0.05).And the tumor lesion diameter in observation group was significantly less than that in control group(P<0.05).Moreover the incidence of adverse reactions in the observation group was significantly lower than that in control group(8.33%vs 21.67%,P<0.05).Conclusion The radioactive particle implantation therapy is of great significance in alleviating disease,improving clinical efficacy and antibody expression.
