摘　　要：目的 分析腹腔镜根治术联合三角吻合术治疗早期胃癌的安全性。方法 将80例早期胃癌患者分为根治术组、联合手术组,每组40例。根治术组采用腹腔镜根治术进行治疗,联合手术组在根治术组基础上增加三角吻合术进行治疗。观察并比较两组患者手术指标、视觉模拟评分法(VAS)评分、消化系统疾病生存质量指数表(GLQL)评分及术后并发症发生情况。结果 联合手术组患者手术时间、住院时间、通气时间均明显短于根治术组,术中出血量明显少于根治术组,淋巴结清扫数量明显多于根治术组,差异均有统计学意义(P﹤0.01)。手术后,两组患者VAS评分均低于手术前,GLQL评分均高于手术前,且联合手术组患者VAS评分低于根治术组,GLQL评分高于根治术组,差异均有统计学意义(P﹤0.05)。两组患者术后并发症总发生率比较,差异无统计学意义(P﹥0.05)。结论 对早期胃癌患者行腹腔镜根治术联合三角吻合术,治疗效果显著,可有效缓解患者临床症状,应用价值较高,可在临床推广使用。Objective To analyze the safety of laparoscopic radical operation combined with triangular anastomosis for early gastric cancer. Method A total of 80 patients with early gastric cancer were divided into radical operation group(n=40, treated with laparoscopic radical operation) and combined operation group(n=40, treated with triangular anastomosis+laparoscopic radical operation). The surgical indexes, scores of visual analog scale(VAS) and gastrointestinal quality of life index(GLQL), postoperative complications were compared between the two groups. Result The operation time, hospitalization time, ventilation time and intraoperative blood loss in the combined operation group were lower and number of lymph node dissection was higher than those in the radical operation group(P<0.01). After operation, the VAS scores in both groups decreased and GLQL increased than those before operation, with lower VAS and higher GLQL scores in the combined operation group than those in the radical operation group(P<0.05). There was no significant difference for the postoperative complications between two groups(P>0.05). Conclusion Laparoscopic radical operation combined with triangular anastomosis for patients with early gastric cancer has significant therapeutic effect, can effectively alleviate the clinical symptoms of patients, has high application value, and can be popularized in clinic.
