摘 要:目的 探讨三时段干预对乳腺癌术中压力性损伤的预防效果。方法 将81例乳腺癌患者按干预方法不同分为对照组41例及观察组40例。对照组患者采用常规干预,观察组患者采用三时段干预。比较两组患者术中情况(手术时间、术中低体温发生率、术中出血量)、术中体温变化情况,同时根据Braden压力性损伤评分结果比较两组患者术中压力性损伤发生情况。结果 观察组患者术中出血量多于对照组,术中低体温发生率低于对照组,差异均有统计学意义(P﹤0.05)。切皮后30 min时、切皮后1 h时、切皮后2 h时及手术结束时,观察组患者体温均明显高于对照组,差异均有统计学意义(P﹤0.01)。观察组患者压力性损伤发生率低于对照组,差异有统计学意义(P﹤0.05)。结论 三时段干预可降低乳腺癌患者压力性损伤发生率及术中低体温发生率,维持患者术中体温。Objective To explore the preventive effect of three-period intervention on stress injury in breast cancer.Method A total of 81 patients with breast cancer were divided into the control group (n=41, received routine intervention) and the observation group (n=40, received three-period intervention). The intraoperative conditions (operation time,incidence of intraoperative hypothermia and intraoperative blood loss) were compared between the two groups. The occurrence of intraoperative stress injury was compared according to the results of Braden stress injury score. Result The intraoperative blood loss in the observation group was higher, incidence of intraoperative hypothermia was lower than those in the control group (P<0.05);the body temperature of the observation group at 30 min, 1 hour, 2 hours after skin incision and at the end of operation in the observation group were significantly higher than those in the control group (P<0.01). The incidence of stress injury in the observation group was significantly lower than that in the control group (P<0.05). Conclusion Three-period intervention can reduce the incidence of stress injury and intraoperative hypothermia in patients with breast cancer, and maintain intraoperative body temperature.
