摘　　要：目的探究胃癌根治术患者术前采用机械性肠道准备(MBP)的必要性及其对感染指标、术后体温的影响。方法选取我院收治的胃癌根治术患者105例,根据随机数字表法分为观察组和对照组。对照组52例术前采用MBP,观察组53例术前不采用MBP,比较两组患者胃肠功能恢复、感染指标[白细胞(WBC)、C反应蛋白(CRP)、中性粒细胞百分比(NEUT)、前白蛋白(PA)]、术后体温及并发症发生情况。结果观察组首次排气时间、首次排便时间短于对照组,手术当日胃肠减压量、持续腹胀、恶心呕吐的发生率均低于对照组(P<0.05)。观察组WBC、CRP、NEUT水平均低于对照组,PA水平高于对照组(P均<0.05)。两组患者术后体温变化比较,P>0.05。观察组并发症发生率低于对照组(P<0.05)。结论术前不采用BMP可有效稳定胃肠道内菌群,减少术后并发症发生率,促进胃肠道功能恢复。Objective To explore the necessity of preoperative MBP for patients undergoing radical gastric cancer surgery and its influence on infection indicators and postoperative body temperature.Methods A total of 105 patients undergoing radical gastric cancer surgery in our hospital were selected and randomly divided into two groups.52 cases in the control group used mechanical bowel preparation(MBP)before operation,and 53 cases in the observation group did not use MBP before operation.The gastrointestinal function recovery,infection indicators[white blood cells(WBC),C-reactive protein(CRP),percentage of neutrophils(NEUT),prealbumin(PA)],postoperative body temperature and complications were compared between the two groups.Results The first exhaust time and first defecation time in the observation group were shorter than those of the control group;the incidence of gastrointestinal decompression,continuous abdominal distension,nausea and vomiting in the observation group were lower than those in the control group(P<0.05).The levels of WBC,CRP,NEUT,in the observation group were lower than those in the control group,PA level was higher than that in the control group(P<0.05).There was no statistically significant change in body temperature between the two groups(P>0.05).The incidence of complications in the observation group was lower than that in the control group(P<0.05).Conclusion Not using BMP before surgery can effectively stabilize the flora in gastrointestinal tract,reduce the incidence of postoperative complications,and promote the recovery of gastrointestinal function.
