摘 要:目的分析以问题为基础的教学(problem-based learning,PBL)结合以案例为基础的教学(case-based learning,CBL)在呼吸与危重症医学科研究生教学中运用的效果。方法选取2019年7月—2021年1月在内蒙古科技大学包头医学院第一附属医院呼吸与危重症医学科(pulmonary and critical care medicine,PCCM)的内科学研究生中随机抽取60名为研究对象,随机分为研究组(n=30)与对照组(n=30),研究组通过PBL联合CBL教学,对照组采用以传统授课为基础的(lecture-based learning,LBL)教学法,教学培训1个月后,通过理论考试和临床实践技能考核比较两组教学效果是否存在差异。结果研究组理论成绩(95.78±4.51)分、临床实践操作技能(92.93±3.11)分均高于对照组研究生的(82.33±4.04)分和(80.48±3.25)分,差异均有统计学意义(P<0.05);问卷调查结果显示研究组学生对PBL联合CBL教学模式对临床思维能力、自主学习能力、文献检索及阅读能力、团队合作能力的提升认可程度均高于对照组,差异均有统计学意义(P<0.05)。结论PBL联合CBL教学模式优于LBL,有助于培养内科学研究生分析、解决临床问题的能力,提高临床思维能力,教学效果更佳。Objective To analyze the effect of problem-based learning(PBL)combined with case-based learning(CBL)in the teaching of postgraduates in respiratory and critical care medicine.Methods 60 postgraduates of internal medicine in the department of pulmonary and critical care medicine(PCCM)of the First Affiliated Hospital of Baotou Medical College of Inner Mongolia University of Science and Technology from July 2019 to January 2021 were randomly selected as the research objects.They were randomly divided into the research group(n=30)and the control group(n=30).The research group was taught by PBL combined with CBL,The control group adopted the teaching based learning(LBL)teaching method.After one month of training,the teaching effects of the two groups were compared through theoretical examination and clinical practice skill assessment.Results The scores of theoretical achievement(95.78±4.51)points and operational clinical practice skill(92.93±3.11)points in the study group were significantly higher than those in the control group(82.33±4.04)points and(80.48±3.25)points(P<0.05).The results of the questionnaire survey showed that the students in the research group recognized the improvement of clinical thinking ability,autonomous learning ability,literature retrieval and reading ability and teamwork ability of PBL combined with CBL teaching mode higher than those in the control group(P<0.05).Conclusion PBL combined with CBL is better than LBL,which is helpful to cultivate the ability of internal science postgraduates to analyze and solve clinical problems,improve clinical thinking ability and the teaching effect is better.
