老年住院患者外周血T淋巴细胞亚群与营养风险的关系

摘　　要：目的探讨老年住院患者外周血T淋巴细胞亚群与营养风险的关系。方法选取2020年8月—2021年9月青岛大学附属医院老年医学科住院的109例老年患者,根据NRS-2002评分将患者分为营养风险组(43例)及营养正常组(66例)。检测两组外周血CD4^(+)T细胞、CD8^(+)T细胞、CD4^(+)/CD8^(+)T细胞比值并进行比较,用Spearman相关、多因素二元logistic回归分析老年住院患者外周血T淋巴细胞亚群与营养风险的关系。结果营养风险组CD4^(+)/CD8^(+)T细胞比值为2.31(1.59,3.06),营养正常组为1.87(1.38,2.40),两组相比P<0.05。老年住院患者CD4^(+)/CD8^(+)T细胞比值与营养风险呈正相关(r=0.200,P=0.037)。多因素logistic回归分析结果显示,CD4^(+)/CD8^(+)T细胞比值是老年住院患者营养风险发生的独立危险因素(OR=2.29,95%CI:1.27~4.13,P=0.006)。结论在有营养风险老年住院患者中,外周血CD4^(+)/CD8^(+)T细胞比值升高,且其是营养风险发生的危险因素。Objective To investigate the relationship between T lymphocyte subsets in peripheral blood and nutri⁃tional risk in elderly inpatients.Methods A total of 109 elderly patients hospitalized in Department of Geriatrics Health⁃care of Affiliated Hospital of Qingdao University from August 2020 to September 2021 were selected.According to the NRS-2002,they were divided into 2 groups:the nutritional risk group(n=43)and the normal nutrition group(n=66).CD4^(+)T cells,CD8^(+)T cells,CD4^(+)/CD8^(+)T cell ratio in peripheral blood of the two groups were measured.Spearman correla⁃tion and multifactor binary logistic regression were used to analyze the relationship between nutritional risk and peripheral blood T lymphocyte subsets in elderly inpatients.Results The radio of CD4^(+)/CD8^(+)T cell was 2.31(1.59,3.06)in the nutritional risk group and 1.87(1.38,2.40)in the normal nutrition group(P<0.05).Nutritional risk was positively cor⁃related with the increased CD4^(+)/CD8^(+)T cell ratio in peripheral blood in elderly inpatients(r=0.200,P=0.037).Logistic regression analysis revealed that CD4^(+)/CD8^(+)T cell radio was an independent factor in prediction of nutritional risk in elder⁃ly inpatients(OR=2.29,95%CI:1.27~4.13,P=0.006).Conclusion CD4^(+)/CD8^(+)T cell ratio increases in elderly in⁃patients with nutritional risk,and it is a risk factor for nutritional risk.
