摘  要:目的探讨F波、H反射及瞬目反射对尿毒症患者近端神经损害的评估价值。方法选择2020年11月至2021年5月在山东省潍坊市坊子区人民医院血液透析中心治疗的尿毒症患者32例,健康对照组17例。检查瞬目反射的潜伏期,上肢正中神经、尺神经和下肢胫后神经F波的出现率和潜伏期,H反射的潜伏期及波幅,并对两组检测结果进行分析。结果瞬目反射检测尿毒症组R1、R2波和R2′波潜伏期均较正常对照组延长,两组比较差异有显著意义(P<0.01)。尿毒症组患者正中神经、胫后神经F波出现率与正常对照组比较差异无统计学意义(P>0.05),尿毒症组尺神经F波出现率降低(P<0.05)。尿毒症组正中神经、尺神经和胫后神经F波潜伏期较正常对照组延长,差异有统计学意义(P<0.05)。H反射潜伏期尿毒症组较正常对照组延长、波幅降低,差异有显著意义(P<0.01)。结论H反射、瞬目反射及尺神经F波等近端周围神经的电生理检查有助于尿毒症周围神经病的诊断。Objective To explore appraised values of F-wave,H-reflex and blink reflex on proximal nerve damage in uremia patients.Methods From Nov.2020 to May 2021,32 uremia patients who were treated in the Hemodialysis Center of Fangzi District People's Hospital and 17 healthy controls were enrolled.Blink reflex latencies,F-wave incidence rates and F-wave latencies of median and ulnar nerves in the upper limb as well as posterior tibial nerve in the lower limb,and the latency and amplitude of H-reflex were examined.Test results in two groups were analyzed.Results The latencies of R1,R2,and R2′waves of blink reflex in uremia group were longer than those in normal control group(P<0.01).There were no statistical differences in the incidence rates of F-waves in median and posterior tibial nerves between uremia and normal control groups(P>0.05).The incidence rate of F wave of ulnar nerve was decreased in uremia group relative to normal group(P<0.05).F-wave latencies of median nerve,ulnar nerve and posterior tibial nerve in uremia group were longer than those in normal control group(P<0.05).When compared with normal control group,the H-reflex latency was prolonged and its amplitude was decreased in uremia group(P<0.01).Conclusion Electrophysiological examination of H-reflex,blink reflex and F wave of proximal peripheral nerves is helpful to diagnose uremia peripheral neuropathy.
