朱氏头皮针联合现代康复治疗对脑卒中痉挛性偏瘫患者平衡功能的影响

摘　　要：目的:探讨朱氏头皮针联合现代康复对脑卒中后痉挛性偏瘫患者平衡功能的影响。方法:将119例脑卒中偏瘫患者随机数字表法按1:1随机分为普通头针组60例和朱氏头皮针组59例,两组患者均使用常规西药、康复及头皮针治疗,普通头针组穴取顶颞前斜线、顶颞后斜线,朱氏头皮针组穴取头面区、下焦区、上肢区、下肢区,两组患者均每日治疗1次,每周6次,共治疗4周。比较2组患者治疗前后的肌张力(改良Ashworth)分级评定、日常生活能力(ADL)评分和平衡功能评测(Fugl-Meyer评测法)评分。结果:治疗4周后,2组患者肌张力分级均较治疗前降低、日常生活能力评分及平衡功能评分均较治疗前升高(P<0.05),2组在提高日常生活能力评分方面无显著差别(P>0.05),但朱氏头皮针组在降低肌张力分级和提高平衡功能评分方面较普通头针组更为显著(P<0.05)。结论:朱氏头皮针联合现代康复治疗能有效改善脑卒中后痉挛性偏瘫患者的肌张力、日常生活能力及平衡功能,且在降低患者肌张力、改善平衡功能方面优于普通头针联合现代康复治疗。Objective:To investigate the effect of Zhu scalp acupuncture combined with modern rehabilitation on balance function of spastic hemiplegia patients after stroke.Methods:A total of 119 s troke patients with hemiplegia were randomly divided into ordinary scalp acupuncture group(n=60)and Zhu scalp acupuncture group(n=59),two groups of patients were used conventional western medicine,rehabilitation and the scalp needle treatment,common group head acupuncture take Anterior parietal temporal oblique line,Posterior parietal temporal oblique line,zhu's scalp acupuncture group take Head and face area,lower focus area,upper limb area and lower limb area,two groups of patients were treatment 1 times a day,the patients were treated 6 times a week for 4 weeks.Muscle tone(modified Ashworth),daily living ability(ADL)and balance function evaluation(FUGL-Meyer)were compared between 2 groups before and after treatment.Results:After 4 weeks of treatment,muscle tension grade of 2 groups decreased,daily living ability score and balance function score increased compared with before treatment(P<0.05),and there was no significant difference in improving daily living ability score between 2 groups(P>0.05).However,zhu scalp acupuncture group was more significant in reducing muscle tension grade and improving balance function score than ordinary scalp acupuncture group(P<0.05).Conclusion:Zhu scalp acupuncture combined with modern rehabilitation therapy can effectively reduce the muscle tension,improve the ability of daily living and balance function of patients with spastic hemiplegia after stroke,and is superior to the ordinary head acupuncture group in reducing the muscle tension and improving balance function.
