摘  要:目的观察健脾益肾活血方对膜性肾病大鼠24 h尿蛋白定量及肾脏组织Nephrin、Podocin蛋白表达的影响。方法将40只SD大鼠随机分为正常组、模型组、贝那普利组、健脾益肾活血组,每组10只。除正常组外,其余组均采用抗FxlA血清一次性尾静脉注射诱导被动型Heymann肾炎模型。造模后第1天开始,贝那普利组给予0.15 mg/mL盐酸贝那普利片悬混液灌胃,健脾益肾活血组给予2.5 g/mL的健脾益肾活血汤灌胃,正常组、模型组给予等量生理盐水灌胃,均1次/d,连续4周。每周末检测各组大鼠24 h尿蛋白定量;于灌胃4周末处死大鼠,心脏取血,检测血清总蛋白、血清白蛋白、血肌酐、总胆固醇、三酰甘油;取肾脏组织,观察病理改变;免疫组化观察肾脏组织中Nephrin、Podocin蛋白阳性表达情况。结果造模后各造模组大鼠24 h蛋白尿定量均明显高于正常组(P均<0.05);灌胃3周和4周后,贝那普利组及健脾益肾活血组大鼠24 h蛋白尿定量均明显低于同期模型组(P均<0.05),健脾益肾活血组与贝那普利组比较差异均无统计学意义(P均>0.05)。末次灌胃结束后,模型组、贝那普利组、健脾益肾活血组血清白蛋白水平和肾脏组织中Nephrin、Podocin表达平均荧光强度均明显低于正常组(P均<0.05),模型组、贝那普利组、健脾益肾活血组血清胆固醇水平和模型组、健脾益肾活血组血清三酰甘油水平均明显高于正常组(P均<0.05),各组血肌酐水平比较差异均无统计学意义(P均>0.05);模型组、贝那普利组及健脾益肾活血组各血清指标水平比较差异均无统计学意义(P均>0.05);贝那普利组、健脾益肾活血组Nephrin与Podocin表达平均荧光强度均明显高于模型组(P均<0.05),且健脾益肾活血组均明显高于贝那普利组(P均<0.05)。结论健脾益肾活血方可以降低膜性肾病大鼠的尿蛋白水平,能够上调肾脏组织中Nephrin、Podocin蛋白的表达。Objective It is to observe the effects of Jianpi Yishen Huoxue Decoction on the 24h urinary protein quantification and the protein expression of Nephrin and Podocin in renal tissues of rats with membranous nephropathy.Methods Forty SD rats were randomly divided into normal group,model group,benazepril group,Jianpi Yishen Huoxue Decoction group,with 10 rats in each group.The rats in all the groups except for the normal group were induced to establish passive Heymann nephritis models by one-time tail vein injection of anti-FxlA serum.From the first day after modeling,the benazepril group was given 0.15 mg/mL benazepril hydrochloride tablet suspension by gavage,the Jianpi Yishen Huoxue group was given 2.5 g/mL Jianpi Yishen Huoxue Decoction by gavage,the normal group and the model group were given the same amount of normal saline by gavage,all once a day,continuously treated for 4 weeks.The 24-hour urine protein quantification of the rats in each group was detected every weekend;the rats were sacrificed at the end of the 4th week of gavage,and their blood was collected from the heart to detect serum total protein,albumin,creatinine,total cholesterol,and triacylglycerol;their kidney tissue was taken for pathological observation.The positive expression of Nephrin and Podocin in kidney tissue was observed by immunohistochemistry.Results After modeling,the 24 h proteinuria of the rats in each modeling group was significantly higher than that in the normal group(all P<0.05);after 3 weeks and 4 weeks of gavage,the 24 h proteinuria in the benazepril group and the Jianpi Yishen Huoxue group were significantly lower than that in the model group at the same time(all P<0.05),and there was no significant difference between the Jianpi Yishen Huoxue group and the benazepril group(P>0.05).After the last intragastric administration,the serum albumin level and the mean fluorescence intensity of Nephrin and Podocin expression in kidney tissue in the model group,benazepril group,and Jianpi Yishen Huoxue group were significantly lower
