顺铂腹腔灌注联合多西他赛静脉给药治疗卵巢癌的有效性及复发率分析

摘　　要：目的探讨在卵巢癌患者的临床治疗中顺铂腹腔灌注联合多西他赛静脉给药对患者治疗效果、免疫功能及复发率的影响。方法选取2018年3月至2019年3月在周口市中心医院接受治疗的120例卵巢癌患者作为研究对象,根据随机数字表法将患者分为两组,分别命名为观察组(n=60)与对照组(n=60),对照组使用多西他赛联合顺铂进行静脉给药治疗,观察组使用多西他赛静脉给药联合顺铂腹腔灌注进行治疗,对两组疗效、免疫功能、血清基质金属蛋白酶-2(MMP-2)和血管内皮生长因子(VEGF)、无进展生存期、病死率、复发率和不良反应发生率进行对比。结果观察组治疗有效率高于对照组,差异有统计学意义(P<0.05);两组治疗后CA125、CD4^(+)CD25^(+)、MMP-2和VEGF水平均明显下降,CD4^(+)/CD8^(+)明显升高,差异有统计学意义(P<0.05),与对照组相比,观察组治疗后CA125、CD4^(+)CD25^(+)、MMP-2和VEGF水平更低,CD4^(+)/CD8^(+)更高,差异有统计学意义(P<0.05);随访2年,观察组无进展生存期明显长于对照组,复发率明显低于对照组,差异有统计学意义(P<0.05),两组的2年病死率和不良反应发生率比较,差异无统计学意义(P>0.05)。结论在卵巢癌患者的临床治疗中,使用顺铂腹腔灌注联合多西他赛静脉给药治疗有着良好的治疗效果,对患者免疫功能的恢复有一定促进作用,可有效控制肿瘤转移和复发、延长患者无进展生存期,且安全性良好。Objective To explore the influences of intraperitoneal infusion of cisplatin combined with docetaxel intravenous administration on curative effect,immune function and recurrence rate in patients with ovarian cancer.Methods A total of 120 patients with ovarian cancer treated in Zhoukou Central Hospital were enrolled as the research objects between March 2018 and March 2019.According to random number table method,they were divided into observation group(n=60)and control group(n=60).The control group was given intravenous administration of docetaxel and cisplatin,while observation group was given intravenous administration of docetaxel combined with intraperitoneal perfusion of cisplatin.The curative effect,immune function,levels of serum matrix metalloproteinase-2(MMP-2)and vascular endothelial growth factor(VEGF),progressionfree survival,mortality,recurrence rate and incidence of adverse reactions were compared between the two groups.Results The total response rate of treatment in observation group was higher than that in control group(P<0.05).After treatment,levels of CA125,CD4^(+)CD25^(+),MMP-2 and VEGF were significantly decreased,while CD4^(+)/CD8^(+)was significantly increased in both groups(P<0.05).Compared with control group,levels of CA125,CD4^(+)CD25^(+),MMP-2 and VEGF in observation group were lower,while CD4^(+)/CD8^(+)was higher(P<0.05).After 2 years of follow-up,progression-free survival in observation group was significantly longer than that in control group,and recurrence rate was significantly lower than that in control group(P<0.05).There was no significant difference in 2-year mortality or incidence of adverse reactions between the two groups(P>0.05).Conclusion In the clinical treatment of patients with ovarian cancer,curative effect of intraperitoneal administration of cisplatin combined with intravenous administration of docetaxel is good,which can promote the recovery of immune function,effectively control tumor metastasis and recurrence,and prolong progression-free survival,with good safety.
