摘　　要：目的:检测长链非编码RNA-GAS5(lncRNA-GAS5)在中低位直肠癌组织中的表达水平及对患者临床病理特征和预后的影响。方法:收集2015年10月—2017年7月在许昌市中心医院接受手术治疗的中低位结直肠癌患者86例,保存患者的肿瘤组织和癌旁组织。qRT-PCR检测组织中lncRNA-GAS5相对表达水平,并根据肿瘤组织中lncRNA-GAS5平均表达水平将患者分为lncRNA-GAS5低表达组和lncRNA-GAS5高表达组。比较两组临床病理特征、3年疾病无进展率和总生存率。Cox单因素和多因素回归分析影响患者死亡的独立危险因素。结果:肿瘤组织中lncRNA-GAS5相对表达水平为1.31±0.21,明显低于癌旁组织的3.66±0.94(t=22.961,P<0.001)。lncRNA-GAS5低表达组40例,lncRNA-GAS5高表达组46例,lncRNA-GAS5表达水平与年龄、TNM分期、浸润深度、淋巴转移和脉管侵犯有关(P<0.05)。lncRNA-GAS5低表达组3年疾病无进展率为25.0%,低于lncRNA-GAS5高表达组的60.9%(χ^(2)=14.601,P<0.001);lncRNA-GAS5低表达组3年总生存率为67.5%低于lncRNA-GAS5高表达组的87.0%(χ^(2)=5.100,P<0.024)。肿瘤分化程度、TNM分期和lncRNA-GAS5表达水平是影响患者死亡的独立危险因素[OR(95%CI):3.774(1.382~3.774)、6.300(4.201~11.632)、2.051(1.847~4.632),均P<0.05]。结论:lncRNA-GAS5在中低位直肠癌中表达水平降低,与患者年龄、TNM分期、浸润深度、淋巴转移和脉管侵犯有关,为影响患者预后的独立危险因素,可作为预后判断的参考指标。Objective:To detect the expression level of long-chain non-coding RNA-GAS5(lncRNA-GAS5)in low-to-middle rectal cancer and its influence on the clinicopathological characteristics and prognosis of patients.Methods:A total of 86 patients with middle and low colorectal cancer who underwent surgery in Xuchang Central Hospital from October 2015 to July 2017 were collected,and the tumor tissues and adjacent tissues of the patients were preserved.qRT-PCR detected the relative expression level of lncRNA-GAS5 in tissues,and divided patients into LncRNA-GAS5 low expression group and lncRNA-GAS5 high expression group according to the average expression level of lncRNA-GAS5 in tumor tissues.The clinicopathological characteristics,3-year disease-free rate and overall survival rate of the two groups were compared.Cox univariate and multivariate regression analysis of independent risk factors affecting patient death.Results:The relative expression level of lncRNA-GAS5 in tumor tissues was 1.31±0.21,which was significantly lower than 3.66±0.94 in adjacent tissues(t=22.961,P<0.001);There were 40 cases in the lncRNA-GAS5 low expression group and 46 cases in the lncRNA-GAS5 high expression group.The expression level of lncRNA-GAS5 was related to the patient's age,TNM stage,depth of invasion,lymphatic metastasis and vascular invasion(P<0.05);The 3-year disease progression-free rate of lncRNA-GAS5 low expression group was 25.0%lower than that of lncRNA-GAS5 high expression group 60.9%(χ^(2)=14.601,P<0.001).The 3-year overall survival rate of lncRNA-GAS5 low expression group was 67.5%lower 87.0%in the lncRNA-GAS5 high expression group(χ^(2)=5.100,P<0.024);Tumor differentiation,TNM staging and lncRNA-GAS5 expression level were independent risk factors affecting patient death[OR(95%CI):3.774(1.382~3.774),6.300(4.201~11.632),2.051(1.847~4.632),all P<0.05].Conclusion:The decreased expression level of lncRNA-GAS5 in low-to-medium rectal cancer is related to the patient′s age,TNM stage,depth of invasion,lymphatic metastasis and vascul
