摘　　要：目的总结腹腔镜下十二指肠乳头肿瘤局部切除的治疗过程,探讨其安全性和可行性。方法回顾性分析四川大学华西医院于2021年6月收治的1例十二指肠乳头神经内分泌肿瘤患者的临床病理资料及手术过程。结果该患者行腹腔镜下十二指肠乳头肿瘤局部切除+原位胆肠及胰肠吻合术。手术持续时间约3 h,出血量约20 mL,术后第3天患者排气。于术后第7天拔出胃管后开始经口进食,于第8天拔出血浆引流管,顺利出院。术后无十二指肠瘘、出血、切口感染等并发症发生。术后随访半年,患者一般情况良好,未见肿瘤复发、转移征象。结论腹腔镜下十二指肠肿瘤局部切除对于累及乳头的良性或低度恶性肿瘤不失为一种合适的选择。Objective To summarize the diagnosis and treatment process of a patient who underwent laparoscopic local excision of duodenal papillary tumor,and to explore the safety and feasibility of this surgery.Method The clinicopathologic characteristics and surgical procedure of the patient with duodenal papillary neuroendocrine tumor admitted to the West China Hospital of Sichuan University in June 2021 were retrospectively analyzed.Results The patient underwent the laparoscopic local excision of duodenal papillary tumor+in situ cholangiojejunostomy and pancreaticojejunostomy.The operation lasted about 3 hours,the blood loss was about 20 mL,and the patient exhausted on the 3rd day after the operation.On the 7th postoperative day,the gastric tube was pulled out and oral feeding was started.On the 8th day,the plasma drainage tube was pulled out and the patient was discharged smoothly.There was no duodenal fistula,bleeding,wound infection,and other complications.After 6 months of follow-up,the general condition of this patient was good,and no tumor recurrence or metastasis was found.Conclusion Laparoscopic local excision is an appropriate option for benign or low-grade malignancies involving the duodenal papillary tumor.
