摘　　要：目的探讨重组组织性纤溶酶原激活剂(rt-PA)静脉溶栓治疗急性缺血性脑卒中(AIS)的疗效及对凝血指标的影响。方法以100例AIS患者为对象,随机分为观察组(rt-PA)与对照组(常规治疗)。对比两组疗效及凝血指标水平。结果观察组总有效率为88.00%(44/50),高于对照组64.00%(32/50)(P <0.05)。两组治疗后活化部分凝血活酶时间(aPTT)、凝血酶原时间(PT)、凝血酶时间(TT)水平升高,观察组aPTT、PT、TT水平高于对照组(P <0.05)。血栓弹力图(TEG)显示:两组治疗R值、K值水平升高,Angle角、MA值水平降低,观察组R值、K值水平高于对照组,Angle角、MA值水平低于对照组(P <0.05)。结论 t-PA治疗AIS的疗效较好,可改善患者的神经功能缺损,纠正机体的凝血功能。Objective To investigate the effect of recombinant tissue plasminogen activator( rt-PA)intravenous thrombolysis in the treatment of acute ischemic stroke( AIS) and its influence on coagulation indexes. Methods 100 patients with AIS were randomly divided into observation group( RT PA) and control group( conventional treatment). The curative effect and coagulation indexes of the two groups were compared. Results The total effective rate of the observation group was 88. 00%( 44/50),which was higher than that of the control group 64. 00%( 32/50)( P < 0. 05). After treatment,the levels of activated partial thromboplastin time( aPTT),prothrombin time( PT) and thrombin time( TT) increased in the two groups,and the levels of aPTT,PT and TT in the observation group were higher than those in the control group( P <0. 05). Thromboelastogram: The R value and K value of the two groups increased,while the angle and Ma value decreased. The R value and K value of the observation group were higher than those of the control group,and the angle and Ma value of the observation group were lower than those of the control group( P <0. 05). Conclusion rt-PA is effective in the treatment of AIS,which can improve the neurological deficit and correct the coagulation function.
