美学区单牙即刻种植即刻修复的牙龈美学分析

摘　　要：目的:研究美学区单牙即刻种植后即刻修复与延期修复两种方案的美学治疗效果,为即刻种植后的修复方式选择提供一定的参考。方法:选择上颌单颗前牙即刻种植的40例患者作为研究对象,根据修复方式不同分为即刻修复和延期修复各20例,然后跟踪随访12个月,对两组患者的粉红美学评分(pink esthetic score,PES)和种植体周围牙槽骨垂直吸收量进行对比分析。结果:通过对两组患者进行12个月的跟踪随访,所有患者依从性好,完全配合医生的治疗和随访,成功率为100%。进行PES评分统计,结果显示即刻修复组PES评分显著高于延期修复组,差异具有统计学意义(P<0.05)。即刻修复组牙槽骨垂直吸收量显著低于延期修复组,差异具有统计学意义(P<0.05)。结论:在严格把握美学区即刻种植的适应证和禁忌证,经过充分与患者沟通,注重手术和修复的细节,患者后期维护得当,美学区单牙即刻种植即刻修复比延期修复可以获得更好的临床效果。Objective To study the aesthetic treatment effects of immediate restoration and delayed restoration of single tooth after immediate implantation in aesthetic area,so as to provide some reference for the selection of restoration methods after immediate implantation.Methods 40 patients with immediate implantation of maxillary single anterior teeth were selected as the research objects.According to different restoration methods,they were divided into 20 cases of immediate restoration and 20 cases of delayed restoration.Then,they were followed up for 12 months.The pink aesthetic score(PES)and the vertical absorption of alveolar bone around the implant were compared and analyzed between the two groups.Results Through 12-month follow-up of the two groups of patients,all patients had good compliance,fully cooperated with the doctor's treatment and follow-up,and the success rate was 100%.The PES score statistics showed that the PES score of the immediate restoration group was significantly higher than that of the delayed restoration group(P<0.05),and the difference was statistically significant.The vertical resorption of alveolar bone in the immediate restoration group was significantly lower than that in the delayed restoration group,and the difference was statistically significant(P<0.05).Conclusion After strictly grasping the indications and contraindications of immediate implant in aesthetic area,fully communicating with patients,paying attention to the details of operation and restoration,and properly maintaining patients in the later stage,immediate implant repair of single tooth in aesthetic area can obtain better clinical effect than delayed restoration.
