合谷穴、太冲穴主治病证及配伍规律的数据挖掘分析

摘　　要：目的通过数据挖掘技术,分析总结得出临床上使用合谷穴、太冲穴配伍治疗疾病的优势病种及腧穴配伍规律,为临床更好地运用两穴治病提供参考依据。方法以中国知网(CNKI)、中文科技期刊数据库(VIP)、万方数据库为检索源,检索年限为1980年1月至2020年12月,根据纳入标准和排除标准筛选文献,建立应用合谷穴、太冲穴相配伍治疗疾病的数据库,运用频次统计、关联规则分析及聚类法分析等分析合谷穴、太冲穴主治的优势病种及与其他腧穴相配伍的规律。结果1980年1月1日至2020年12月31日期间临床运用合谷穴、太冲穴或者以合谷穴、太冲穴为主穴与其他腧穴相配伍可治疗74种疾病,优势病种分别是面瘫、头痛、郁证等。病种涉及多个系统,频次由高到低前3位分别是神经系统疾病、精神障碍疾病、妇科疾病等。对配伍腧穴进行关联规则分析,发现临床上与合谷穴、太冲穴配伍治疗疾病的核心配穴是百会、印堂、风池、太阳等。配穴归经前3位分别是奇穴、足阳明胃经、足太阳膀胱经等。对穴位刺激方法进行分析发现,以毫针针刺、电针及针药结合为主。结论临床应用合谷穴、太冲穴或以合谷穴、太冲穴为主配伍其他腧穴治疗优势病种为面瘫、头痛、郁证等神经系统疾病及精神障碍类疾患。Objective:To analyze and summarize the suitable diseases and the compatibility laws of Hegu(LI4and Taichong(LR3)through data excavation,and to provide reference for clinical better use of the two acupoints.Methods:Databases such as CNKI,VIP,and Wanfang were searched from January 1980 to December 2020.Literatures were screened according to the inclusion and exclusion criteria,and the application Hegu(LI4)and Taichong(LR3)for treating diseases was established.using Frequency statistics,association rule analysis and clustering analysis were used to analyze the suitable diseases of Hegu(LI4)and Taichong(LR3)acupoints and the rules of compatibility with other acupoints.Results:From January 1,1980 to December 31,2020,74 diseases were treated with Hegu(LI4)and Taichong(LR3),or other acupoints with Hegu(LI4)and Taichong(LR3)as the main ones.The suitable diseases were facial paralysis,headache and depression.Diseases involved multiple systems,and the top three were nervous system diseases,mental disorders,gynecological diseases,ranking from high to low.The compatibility rules of the acupoints were analyzed and found that the core matching acupoints of the diseases with Hegu(LI4)and Taichong(LR3)were Baihui(DU20),Yintang(EX-HN3),Fengchi(GB20)and Taiyang(EXHN5).The top three were extra meridian acupoints,the stomach meridian of foot Yangming and the bladder meridian of foot Taiyang.Through the analysis of acupoint stimulation method,it showed that the main treatment methods were acupuncture,electroacupuncture and the combination of acupuncture and medicine.Conclusion:The suitable diseases treated by Hegu(LI4)and Taichong(LR3)or Hegu(LI4)and Taichong(LR3)as the main acupoints with other ones are facial paralysis,headache,depression and other neurological diseases and mental disorders.
