摘 要:目的探讨不同照射野数调强放疗对照射区体部剂量分布的影响。方法选择2018年2月至2021年3月该院收治的30例直肠癌术后调强放疗患者,在治疗计划系统中,为每例患者设计3套调强放疗计划:一套5个治疗野、一套7个治疗野、一套9个治疗野。根据每例患者的5、10、15、20、25、30、35、40、45、50 Gy的剂量曲线,计算出其所包含的体部体积V5、V10、V15、V20、V25、V30、V35、V40、V45、V50,然后将3套调强治疗计划分别进行比较。采用SPSS19.0统计软件包进行统计分析。结果当剂量为10、15 Gy时,剂量曲线所包含体积均值随射野数的增加而增加,差异有统计学意义(P<0.05)。当剂量为20、25、30、35、40、45 Gy时,剂量曲线所包含体积均值随射野数的增加而减小,差异有统计学意义(P<0.05)。结论直肠癌术后5、7、9个不同照射野数调强放疗,当剂量在10~20 Gy时,剂量曲线所包含体积均值随射野数的增加而增加;当剂量大于20 Gy时,剂量曲线所包含体积均值随射野数的增加而减小。Objective To investigate the effect of the intensitymodulated radiotherapy with different number of fields on the dose distribution in the body of the irradiated area.Methods A total of 30 patients were selected for the post-operative intensity-modulated radiotherapy for rectal cancer,and three sets of intensity-modulated radiotherapy plans were designed for each patient in the treatment planning system:one set of five treatment fields,one set of seven treatment fields and one set of nine treatment fields.Based on the dose profile of 5,10,15,20,25,30,35,40,45 and 50 Gy for each patient,the volume of the body part included V5,V10,V15,V20,V25,V30,V35,V40,V45 and V50 were calculated and then the three sets of intensity-modulated treatment plans were compared separately.SPSS19.0 statistical package was used to perform the statistical analysis.Results At the dose of 10 and 15 Gy,the mean value of the volume included in the dose curve increased with the number of fields,with a statistically significant difference(P<0.05).At the dose of 20,25,30,35,40 and 45 Gy,the mean value of the volume included in the dose curve decreased with the increasing number of the fields.The differences were statistically significant(P<0.05).Conclusion Intensity modulated radiotherapy with five,seven and nine different irradiation fields for rectal cancer after operation.When the dose was between 10-20 Gy,the volume average included in the dose curve increased with the increase of the irradiation fields.When the dose was greater than 20 Gy,the volume average included in the dose curve decreased with the increase of the number of the fields.
