免疫球蛋白联合糖皮质激素冲击疗法对重症肌无力患者血清AChR-Ab水平和临床症状的影响

摘　　要：目的探究免疫球蛋白联合糖皮质激素冲击疗法对重症肌无力患者血清抗乙酰胆咸受体抗体(AChR-Ab)水平和临床症状的影响。方法方便选取该院2016年4月—2021年3月收治的80例重症肌无力患者,将其随机按照每组40例患者分为对照组和观察组。其中对照组采用糖皮质激素冲击疗法,观察组在使用糖皮质激素冲击疗法基础上加用免疫球蛋白进行治疗,对比两组患者治疗后血清AChR-Ab水平、治疗有效率、临床症状积分和不良反应发生情况。结果观察组患者的血清AChR-Ab水平为(0.21±0.10)nmol/L,显著低于对照组(0.37±0.19)nmol/L,差异有统计学意义(t=4.710,P<0.05);观察组的治疗有效率为95.00%,明显高于对照组(75.00%),差异有统计学意义(χ^(2)=6.270,P<0.05);观察组的临床症状积分为(4.96±1.62)分,显著低于对照组(8.03±2.45)分,差异有统计学意义(t=6.610,P<0.05);观察组患者的不良事件发生率为0.00%,显著低于对照组(20.00%),差异有统计学意义(χ^(2)=6.800,P<0.05)。结论免疫球蛋白联合糖皮质激素冲击疗法在治疗重症肌无力患者中可以提高疗效,缓解患者不适感,在临床治疗中安全性能较好。Objective To explore the effect of immunoglobulin combined with glucocorticoid pulse therapy on serum anti-acetylcholine receptor antibody-Ab(AChR-Ab)levels and clinical symptoms in patients with myasthenia gravis.Methods Conveniently selected 80 patients with myasthenia gravis who were admitted to the hospital from April 2016 to March 2021,and randomly divided them into a control group and an observation group according to 40 patients in each group.The control group was treated with glucocorticoid shock therapy,and the observation group was treated with immunoglobulin on the basis of glucocorticoid shock therapy.The serum AChR-Ab levels,treatment effective rate,clinical symptom scores and occurrence of adverse reactions were compared between the two groups of patients after treatment.Results The serum AChR-Ab level of the observation group was(0.21±0.10)nmol/L,which was significantly lower than that of the control group(0.37±0.19)nmol/L,the difference was statistically significant(t=4.710,P<0.05).The effective rate of treatment in the observation group was 95.00%,which was significantly higher than that in the control group(75.00%),the difference was statistically significant(χ^(2)=6.270,P<0.05).The clinical symptom score of the observation group was(4.96±1.62)points,which was significantly lower than the control group(8.03±2.45)points,the difference was statistically significant(t=6.610,P<0.05).The incidence of adverse events in the observation group was 0.00%,which was significantly lower than that in the control group(20.00%),the difference was statistically significant(χ^(2)=6.800,P<0.05).Conclusion The immunoglobulin combined with glucocorticoid pulse therapy can improve the curative effect in the treatment of patients with myasthenia gravis,relieve the discomfort of the patients,and have better safety performance in clinical treatment.
