血清中N端骨钙素、神经膜蛋白2表达水平与鼻咽癌预后的相关性及相关因素分析

摘　　要：目的 探讨血清中N端骨钙素(N-Osrteoc)、神经膜蛋白2(NRSN2)表达水平与鼻咽癌预后的相关性及相关因素。方法 回顾性选取2019年4月至2020年10月海南西部中心医院收治鼻咽癌患者63例作为研究对象。采用电化学发光法检测血清N-Osrteoc水平,采用荧光定量聚合酶链式反应检测血清NRSN2水平。观察鼻咽癌患者治疗前后和健康对照组血清N-Osrteoc、NRSN2水平表达。采用Pearson相关性分析血清N-Osrteoc、NRSN2水平表达与鼻咽癌的相关性,并绘制受试者工作特征(ROC)曲线分析血清N-Osrteoc、NRSN2对鼻咽癌的诊断价值。结果 鼻咽癌组治疗前血清N-Osrteoc、NRSN2水平均显著高于鼻咽癌组治疗后、对照组,差异均有统计学意义(P <0. 05)。血清N-Osrteoc、NRSN2水平变化与鼻咽癌及预后呈正相关(r=0. 458,0. 574,均P <0. 001)。建立ROC曲线,血清N-Osrteoc、NRSN2 AUC分别为0. 735、0. 758,提示血清N-Osrteoc、NRSN2对鼻咽癌预后具有一定的诊断价值。年龄<45岁鼻咽癌患者治疗后血清N-Osrteoc水平低于≥45岁患者;TNM分期Ⅰ~Ⅱ期患者治疗前后血清N-Osrteoc、NRSN2水平低于Ⅲ~Ⅳ期患者;未见于骨转移患者治疗前后血清N-Osrteoc水平低于骨转移患者,未见于骨转移患者治疗后NRSN2低于骨转移者;KPS评分> 70分患者治疗后血清N-Osrteoc水平低于≤70分患者,KPS评分> 70分患者治疗前后NRSN2水低于≤70分患者,差异均有统计学意义(P <0. 05)。TNM分期、骨转移、病理类型、KPS评分是影响鼻咽癌患者治疗后血清N-Osrteoc、NRSN2水平变化的相关因素,年龄是影响鼻咽患者治疗后血清N-Osrteoc水平变化的相关因素。结论 血清中N-Osrteoc、NRSN2表达水平与鼻咽癌预后具有相关性,受年龄、TNM分期、骨转移、不同病理特征、KPS评分等因素影响。Objective To investigate the relationship between the expression of N-Osrteoc and neurensin-2(NRSN2)in serum and the prognosis of nasopharyngeal carcinoma(NPC).Methods Sixty-three patients with nasopharyngeal carcinoma admitted to Hainan West Central Hospital from April 2019 to October 2020 were retrospectively selected as research object.Serum N-Osrteoc levels were measured by electrochemiluminescence and serum NRSN2 levels were measured by fluorescence quantitative polymerase chain reaction.The expression of serum N-Osrteoc and NRSN2 levels in NPC patients and healthy controls was observed,and the correlation between the expression of serum N-Osrteoc and NRSN2 levels and NPC was analyzed using Pearson correlation.Receiver operating characteristic(ROC)curves were plotted to analyze the diagnostic value of serum N-Osrteoc and NRSN2 for NPC.Results The levels of serum N-Osrteoc and NRSN2 in the nasopharyngeal carcinoma group before treatment were significantly higher than those in the nasopharyngeal carcinoma group after treatment and in the control group,and the differences were statistically significant(P<0.05).The changes of serum N-Osrteoc and NRSN2 levels were positively correlated with nasopharyngeal carcinoma and prognosis(r=0.458,0.574,both P<0.001).ROC curves were established.The AUCs of serum N-Osrteoc and NRSN2 were 0.735 and 0.758,respectively,suggesting that serum N-Osrteoc and NRSN2 had certain diagnostic value for NPC prognosis.The serum N-Osrteoc level of NPC patients aged<45 years after treatment was lower than that of patients aged≥45 years.The serum N-Osrteoc and NRSN2 levels of TNM stageⅠ-Ⅱpatients before and after treatment were lower than those of patients with stageⅢ-Ⅳ.The serum N-Osrteoc level of patients without bone metastasis before and after treatment was lower than that of patients with bone metastasis,and so was the NRSN2 level.The pathological serum N-Osrteoc and NRSN2 levels of patients with different pathological characteristics were significantly different.The serum N-Os
