摘  要:目的:探讨儿童抽动障碍(TD)与患儿血清铁(SI)、血清铁蛋白(SF)及血清25羟维生素D[25(OH)D]水平的相关性,为发现TD的发病机制提供依据。方法:采取病例对照研究,选取2019年1月-2021年10月内蒙古自治区妇幼保健院儿内科治疗的TD患儿54例为试验组;选择同期年龄、性别与试验组保持均衡的54例于本院体检的健康儿童作为对照组。采用直线相关分析试验组25(OH)D、SI、SF水平与耶鲁抽动症整体严重度量表(YGTSS)评分的相关性。结果:试验组SI、SF、25(OH)D水平均低于对照组,差异均有统计学意义(P<0.05),试验组与对照组25(OH)D正常、不足、缺乏的比例分布比较,差异有统计学意义(P<0.05)。暂时性抽动障碍(TTD)组、慢性抽动障碍(CTD)组、Tourette综合征(TS)组和对照组的SI、SF、25(OH)D水平及25(OH)D正常、不足、缺乏的比例分布比较,差异均有统计学意义(P<0.05)。YGTSS评分与25(OH)D、SI、SF水平均呈显著的负相关(r=-0.410、-0.489、-0.394,P<0.01)。结论:血清铁降低、血清铁蛋白减少可能导致儿童抽动障碍的发生,且二者水平越低,临床多表现为CTD和TS,TD患儿的SI、SF、25(OH)D水平越低,YGTSS评分越高。Objective:To investigate the correlation between tic disorder (TD) in children and serum iron (SI),serum ferritin (SF) and serum 25-hydroxyvitamin D[25 (OH) D]levels,to provide a basis for discovering the pathogenesis of TD.Method:A case-control study was conducted in this study,fifty-four TD children treated in the Pediatric Department of Inner Mongolia Maternal and Child Health Hospital from January 2019 to October 2021 were selected as the experimental group.In the same period,54 healthy children whose age and gender were balanced with the experimental group in our hospital for physical examination were selected as the control group.Linear correlation was used to analyze the correlation between 25(OH)D,SI,SF levels and YGTSS scores in the experimental group.Result:The levels of SI,SF and 25(OH)D in the experimental group were lower than those in the control group,the differences were statistically significant (P<0.05).There were significant differences in the proportion of normal,insufficient and deficient 25(OH)D levels between the experimental group and the control group (P<0.05).There were significant differences in the levels of SI,SF,25(OH)D and the proportion of normal,insufficient and deficient 25(OH)D in transient tic disorders (TTD),chronic tic disorder (CTD),Tourette syndrome (TS) and control group (P<0.05).There were significant negative correlations between YGTSS score and 25(OH)D,SI and SF levels (r=-0.410,-0.489,-0.394,P<0.01).Conclusion:The decrease of serum iron and serum ferritin may lead to the occurrence of tic disorder in children,and the lower the levels of serum iron and serum ferritin,the more clinical manifestations of CTD and TS,and the lower the levels of SI,SF and 25(OH)D in children with TD,the higher the scores of YGTSS.
