无牙颌患者睡眠佩戴义齿对阻塞性睡眠呼吸暂停综合征的影响

摘　　要：目的:探究无牙颌患者睡眠佩戴义齿对阻塞性睡眠呼吸暂停综合征(OSAS)的影响。方法:选取2019年2月至2021年1月肇庆市第一人民医院口腔科收治的无牙颌患者112例作为研究对象,按照是否佩戴义齿睡眠分为观察组(佩戴义齿)和对照组(未佩戴义齿),每组56例。分析无牙颌患者睡眠佩戴义齿睡眠对呼吸暂停低通气指数(AHI)、睡眠中低氧饱和度(L-SPO2)、收缩压(SBP)、舒张压(DBP)及OSAS的影响。结果:观察组患者AHI指数明显高于对照组(P<0.05),2组患者L-SPO2、SBP、DBP比较,差异均无统计学意义(均P>0.05);56例患者有50例AHI增加,6例AHI减少;观察组患者的口臭、义齿性口腔炎、创伤性溃疡、颞下颌关节紊乱例数均高于对照组,差异均有统计学意义(均P<0.05)。结论:无牙颌患者睡眠佩戴义齿睡眠可增加AHI指数,增加义齿性口腔炎、颞下颌关节紊乱等疾病发病的风险,从而增加OSAS的风险。Objective:To investigate the influence of edentulous jaw patients on obstructive sleep apnea syndrome(OSAS).Methods:A total of 112 patients with edentulous jaw admitted to the First People′s Hospital of Zhaoqing City from February 2019 to January 2021 were selected as subjects.They were divided into observation group(with denture)and control group(without denture),with 56 cases in each group.The effects of denture wearing on apnea-hypopnea index(AHI),low oxygen saturation(L-SPO2),systolic blood pressure(SBP),diastolic blood pressure(DBP)and OSAS in patients with edentulous jaw were analyzed.Results:The AHI index in the observation group was significantly higher than that in the control group(P<0.05),there was no significant difference in L-SPO2,SBP and DBP between the two groups(Ps>0.05).AHI increased in 50 of the 56 patients and decreased in 6 cases.The cases of stench,denture stomatitis,traumatic ulcer and temporomandibular joint disorder in the observation group were higher than those in the control group,and the difference was statistically significant(Ps<0.05).Conclusion:Edentulous patients wearing dentures during sleep can increase the risk of AHI index and diseases such as denture stomatitis and temporomandibular joint disorder,thereby increasing the risk of OSAS.
