摘  要:目的:探讨25羟维生素D[25 hydroxyvitamin D,25-(OH)D]及脂蛋白相关磷脂酶A2(lipoprotein-associated phospholipase A2,Lp-PLA2)水平与中年高血压人群颈动脉斑块形成的关系。方法:选取锦州医科大学附属第一医院2019年7月-2021年10月就诊的100例原发性高血压合并颈动脉斑块形成的中年患者(40岁≤年龄<60岁)作为斑块组,另选取100例单纯原发性高血压中年患者作为对照组。比较两组临床资料,分析中年高血压人群颈动脉斑块形成的影响因素;分析25-(OH)D和Lp-PLA2预测中年高血压患者斑块形成的价值;分析25-(OH)D与Lp-PLA2的相关性。结果:斑块组年龄、吸烟史、高血压病程、空腹血糖(fasting plasma glucose,FPG)、同型半胱氨酸(homocysteine,Hcy)、总胆固醇(total cholesterol,TC)、Lp-PLA2均高于对照组(P<0.05)。斑块组高密度脂蛋白胆固醇(high-density lipoprotein cholesterol,HDL-C)、25-(OH)D均低于对照组(P<0.05)。两组性别、饮酒史、体重指数(body mass index,BMI)、高血压分级、甘油三酯(triglyceride,TG)、尿酸(uric acid,UA)、低密度脂蛋白胆固醇(low-density lipoprotein cholesterol,LDL-C)比较,差异均无统计学意义(P>0.05)。年龄、TC、Lp-PLA2是中年高血压患者斑块形成的独立危险因素,HDL-C、25-(OH)D是保护因素(P<0.05)。25-(OH)D、Lp-PLA2预测斑块形成的最佳截断值分别为23.84 ng/mL,424.45 U/L,曲线下面积(AUC)分别是0.927、0.916。25-(OH)D与Lp-PLA2呈负相关(P<0.05)。结论:年龄、TC和Lp-PLA2是中年高血压人群颈动脉斑块形成的独立危险因素,HDL-C、25-(OH)D是其保护因素。25-(OH)D、Lp-PLA2对中年高血压人群颈动脉斑块形成的预测价值较高,且二者呈负相关。临床中应加强对中年高血压人群25-(OH)D和Lp-PLA2的监测,有利于早期发现颈动脉斑块形成。Objective:To investigate the relationship between 25 hydroxyvitamin D[25 hydroxyvitamin D,25-(OH)D]and lipoprotein-associated phospholipase A2(Lp-PLA2)levels and carotid artery plaque formation in middle-aged hypertensive patients.Method:A total of 100 middle-aged patients(40≤age<60 years old)with essential hypertension complicated with carotid plaque in the First Affiliated Hospital of Jinzhou Medical University from July 2019 to October 2021 were selected as the plaque group,and 100 middle-aged patients with simple essential hypertension were selected as the control group.The clinical data of the two groups were compared to analyze the influencing factors of carotid plaque formation in middle-aged hypertension patients.The value of 25-(OH)D and Lp-PLA2 in predicting plaque formation in middle-aged hypertensive patients was analyzed.The correlation between 25-(OH)D and Lp-PLA2 was analyzed.Result:Age,smoking history,hypertension course,fasting plasma glucose(FPG),homocysteine(Hcy),total cholesterol(TC)and Lp-PLA2 were higher than those in control group(P<0.05).High-density lipoprotein cholesterol(HDL-C)and 25-(OH)D in plaque group were lower than those in control group(P<0.05).There were no statistically significant differences between the two groups in gender,drinking history,body mass index(BMI),hypertension grade,triglyceride(TG),uric acid(UA),low-density lipoprotein cholesterol(LDL-C)(P>0.05).Age,TC and Lp-PLA2 were independent risk factors for plaque formation in middle-aged hypertensive patients,while HDL-C and 25-(OH)D were protective factors(P<0.05).The best truncation values of 25-(OH)D and Lp-PLA2 for predicting plaque formation were 23.84 ng/mL and 424.45 U/L,respectively,and the area under curve(AUC)was 0.927 and 0.916,respectively.25-(OH)D was negatively correlated with Lp-PLA2(P<0.05).Conclusion:Age,TC and Lp-PLA2 are independent risk factors for carotid plaque formation in middle-aged hypertensive patients,while HDL-C and 25-(OH)D are protective factors.25-(OH)D and Lp-PLA2 have higher predictiv
