摘　　要：目的:比较ⅢB期非小细胞肺癌微创手术与开放手术的临床效果。方法:研究对象为80例ⅢB期非小细胞肺癌患者,按照入院先后顺序分为两组,每组40例。对照组采用开放手术治疗,观察组实施微创手术治疗,比较不同手术方法应用效果。结果:观察组手术术中出血量、术后引流量、置管时间以及住院时间、并发症发生率少于对照组(P<0.05),术后的CD_(3)^(+)、CD_(4)^(+)、CD_(8)^(+)细胞分数高于对照组(P<0.05),淋巴结清除率观察组与对照组未见明显差异。结论:ⅢB期非小细胞肺癌采用微创手术治疗,能够保证淋巴结清除率、且造成的创伤小、住院时间短、并发症发生率少,对机体免疫功能造成的影响小。Objective:To compare the clinical outcomes of VATS surgery and open surgery for stage Ⅲ B non-small cell lung cancer.Methods:80 patients with stage ⅢB non-small cell lung cancer were divided into observation group(n=40)and reference group(n=40),the reference group was treated with open surgery,and the observation group was treated with VATS surgery,to compare the effects of different surgical methods.Results:There was significant difference between the observation group and the reference group in the intraoperative blood loss,postoperative drainage,the drainage tube indwelling time,hospital stay,complication rate and postoperative CD_(3)^(+)、CD_(4)^(+)、CD_(8)^(+)cell scores(P<0.05).There was no significant difference between the lymph node clearance rate observation group and the control group.Conclusion:The VATS surgery for stage ⅢB non-small cell lung cancer can ensure the clearance rate of lymph nodes,and less trauma,shorter hospital stay,less complication rate,and less impact on immune function.
