CIK/DC-CIK联合化疗治疗多发性骨髓瘤的Meta分析

摘　　要：目的评价CIK或DC-CIK联合化疗治疗多发性骨髓瘤的疗效。方法两名评价员以同种方法检索相关数据库,按纳入及排除标准筛选出2017年11月之前发表的关于CIK或DC-CIK联合化疗治疗多发性骨髓瘤的随机对照研究,采用随机或固定效应模型进行性了汇总分析。结果共纳入15篇文献,779例患者。与单独化疗组相比,CIK/DC-CIK免疫疗法联合化疗组显著提高了患者的ORR(P=0.0007)、PS评分及白蛋白均显著升高(P<0.00001),而浆细胞比例及尿轻链显著降低(P<0.00001)。同时CD3^(+)CD4^(+)亚群、IL-2水平及AgNORS显著增加(P<0.00001),CD3^(+)CD8^(+)、CD4^(+)CD25+/CD4^(+)、CD4^(+)CD25+FOXP3+/CD4^(+)CD25+与TGF-β较单纯化疗组显著降低(P<0.00001)。结论CIK/DC-CIK免疫治疗联合化疗与单独化疗治疗多发性骨髓瘤比较,能够显著提高治疗效率,增强患者免疫功能。Objective To evaluate the efficacy of CIK or DC-CIK combined with chemotherapy in the treatment of multiple myeloma.Methods The two evaluators searched the relevant databases with the same method,screened out the randomized controlled studies on the treatment of multiple myeloma with CIK or DC-CIK combined chemotherapy published before November 2017 according to the inclusion and exclusion criteria,and made a summary analysis with random or fixed effect model.Results A total of 15 literatures and 779 patients were included.Compared with chemotherapy alone,CIK/DC-CIK immunotherapy combined with chemotherapy significantly increased ORR(P=0.0007),PS score and albumin(P<0.00001),while the proportion of plasma cells and urinary light chain decreased significantly(P<0.00001).At the same time,the levels of CD3^(+)CD4^(+)subsets,IL-2 and AgNORs increased significantly(P<0.00001).CD3^(+)CD8^(+),CD4^(+)CD25+/CD4^(+),CD4^(+)CD25+Foxp3+/CD4^(+)CD25+and TGF-βwere significantly lower than that in the simple chemotherapy group(P<0.00001).Conclusion Compared with chemotherapy alone,CIK/DC-CIK immunotherapy combined with chemotherapy can significantly improve the treatment efficiency and enhance the immune function of patients with multiple myeloma.
