奥扎格雷钠治疗脑梗死有效性、安全性及经济性的卫生技术评估

摘　　要：目的评估奥扎格雷钠治疗脑梗死的有效性、安全性和经济性,为临床合理用药及决策者做出决策提供循证依据。方法系统检索PubMed、Embase、Cochrane Library、中国知网、维普、万方和卫生技术评估(HTA)相关数据库及网站,纳入奥扎格雷钠对比安慰剂或其他药物治疗脑梗死的HTA报告、系统评价或Meta分析及药物经济学研究。由两名研究者独立按照纳入和排除标准筛选文献,评价文献质量,提取数据并汇总分析,交叉核对,有分歧相互讨论及咨询第三方。结果共纳入HTA报告0篇,Meta分析14篇和药物经济学研究18篇。结果显示,奥扎格雷钠的有效性比安慰剂、阿司匹林、舒血宁更好。奥扎格雷钠分别联合舒血宁、依达拉奉、阿司匹林、疏血通、丹红、银杏达莫、灯盏细辛(灯盏花素)与单用各药相比,联用有效性更好。奥扎格雷钠与依达拉奉有效性相当。奥扎格雷钠联合阿司匹林与低分子肝素、氯吡格雷、西洛他唑、银杏达莫、阿托伐他汀分别联合阿司匹林相比,有效性相当。奥扎格雷联合依达拉奉与尤瑞克林联合依达拉奉有效性相当。奥扎格雷比阿加曲班有效性差。奥扎格雷的安全性与对照药物基本相当。在经济性方面,与依达拉奉、丹红、前列地尔、血塞通、灯盏花素、丹参、丁咯地尔、长春西汀、银杏达莫、阿魏酸钠、葛根素联合吡拉西坦、前列腺素、川芎嗪、巴曲酶、三七总皂苷、疏血通相比,奥扎格雷钠不具有经济性优势。与丁苯酞联合阿司匹林、丁苯酞注射液联合丁苯酞软胶囊相比,奥扎格雷钠联合阿司匹林不占经济性优势。与参麦、尿激酶相比,奥扎格雷具有经济性优势。与银杏达莫联合依达拉奉、阿加曲班联合依达拉奉相比、奥扎格雷钠联合依达拉奉较为经济。结论奥扎格雷钠治疗脑梗死具有较好的有效性、安全性,经济性不占优势。Objective To evaluate the effectiveness,safety and economy of ozagrel sodium in the treatment of cerebral infarction,so as to provide evidence-based basis for clinical rational drug use and decision makers.Methods PubMed,Embase,Cochrane Library,CNKI,VIP,Wanfang and health technology assessment(HTA)related databases and websites were systematically searched,and the HTA reports,systematic reviews,meta-analyses and pharmacoeconomic studies of ozagrel sodium versus placebo or other drugs in the treatment of cerebral infarction were included.According to the inclusion and exclusion criteria,two researchers independently screened the literatures,evaluated the quality of the literatures,extracted the data,summarized and analyzed,then cross-checked,discussed with each other,and consulted the third party if there were any differences.Results Totally 0 HTA reports,14 Meta-analyses and 18 pharmacoeconomic studies were included.The results showed that ozagrel sodium was more effective than placebo,aspirin,and Shuxuening.Ozagrel sodium combined with Shuxuening,edaravone,aspirin,Shuxuetong,Danhong,ginkgo damol,erigeron breviscapus(breviscapine)showed better efficacy than single use of each drug.Ozagrel sodium is as effective as edaravone.The efficacy of ozagrel sodium combined with aspirin was similar to that of low molecular weight heparin,clopidogrel,cilotazol,ginkgo damol and atorvastatin combined with aspirin.Ozagrel combined with edaravone is as effective as urinary kallidinogenase combined with edaravone.Ozagrel is less effective than agtroban.The safety of ozagrel was similar to that of control drugs.Compared with edaravone,Danhong,alprostadil,Xuesaitong,breviscapine,Danshen,buflomedil,vinpocetine,ginkgo damo,sodium ferulate,puerarin combined with piracetam,prostaglandin,ligustrazine,batroxobin,panax notoginseng saponins and Shuxuetong,ozagrel sodium has no economic advantage.Compared with butylphthalide combined with aspirin and butylphthalide injection combined with butylphthalide soft capsule,ozagrel sodium combined
