摘　　要：原发性肝癌作为中国发病率第5位以及致死率第2位的恶性肿瘤,严重威胁着中国人民的生命健康。目前临床仍面临着肝癌的手术切除率较低以及术后复发率过高的难题,寻求有效且可靠的围手术期治疗方案以提高疗效至关重要。近年来,应用FOLFOX方案的肝动脉灌注化疗(HAIC)及其联合治疗显著提高了肿瘤反应率和患者生存率,应用也越来越广泛。近年来的研究显示,将HAIC应用于肝癌手术前的降期转化和术后辅助治疗,在提高手术切除率、降低术后复发风险以及改善患者长期预后等方面起到了积极的作用。该文将对HAIC在肝癌围手术期的临床应用和研究进展进行综述,以期为肝癌围手术期的治疗提供借鉴意义。Primary liver cancer(refers to hepatocellular carcinoma)is currently the fifth most common malignant tumor and the second most lethal cause of cancer in China,seriously threatening the lives and health of Chinese people.Low resection rate and high postoperative recurrence rate of liver cancer are urgent problems to be solved in clinic.It is of great importance to seek effective perioperative treatment to improve surgical efficacy.In recent years,FOLFOX regimen of hepatic arterial infusion chemotherapy(HAIC)and its combination therapy have significantly improved the tumor response rate and patient survival rate,and the application is becoming more and more widespread.Studies in recent years have shown that HAIC,as an effective means of preoperative descending transformation and postoperative adjuvant therapy for liver cancer,plays a positive role in improving the surgical resection rate,reducing the risk of postoperative recurrence and improving the long-term prognosis of patients.This paper will review the clinical application and research progress of HAIC in the perioperative period of liver cancer,in order to provide reference for perioperative treatment of hepatocellular carcinoma.
