内镜下窄带成像联合常规内镜检查在早期胃癌诊断中的效能

摘　　要：目的:探讨内镜下窄带成像(NBI)联合常规内镜检查在早期胃癌诊断中的效能。方法:选取80例疑似早期胃癌患者为研究对象,所有患者均接受常规内镜与NBI检查,以病理组织学检查结果为“金标准”,分析NBI联合常规内镜检查在早期胃癌诊断中的效能。结果:病理检查结果显示,80例疑似早期胃癌患者中恶性69例,良性11例;常规内镜检查结果显示,恶性35例,良性45例;NBI联合常规内镜检查结果显示,恶性59例,良性21例;NBI联合常规内镜检查诊断早期胃癌的准确度、灵敏度、阴性预测值均高于常规内镜检查,差异有统计学意义(P<0.05);NBI与常规内镜检查诊断早期胃癌的特异度、阳性预测值比较,差异均无统计学意义(P>0.05)。结论:NBI联合常规内镜检查诊断早期胃癌的效能高于常规内镜检查。Objective:To investigate efficacy of endoscopic narrow band imaging(NBI)combined with conventional endoscopy in diagnosis of early gastric cancer.Methods:80 patients with suspected early gastric cancer were selected as the research objects,and all of them underwent routine conventional and NBI examination.Using the results of histopathological examination as the“gold standard”,the efficacy of NBI combined with conventional endoscopy in the diagnosis of early gastric cancer was analyzed.Results:The pathological examination results showed that among the 80 patients with suspected early gastric cancer,69 were malignant and 11 were benign.The accuracy,sensitivity and negative predictive value of NBI combined with conventional endoscopy in the diagnosis of early gastric cancer were higher than those of conventional endoscopy,and the differences were statistically significant(P<0.05).However,there were no significant differences in the specificity and the positive predictive value of NBI and conventional endoscopy in the diagnosis of early gastric cancer(P>0.05).Conclusions:The diagnostic efficiency of NBI combined with conventional endoscopy in the diagnosis of early gastric cancer is higher than that of conventional endoscopy.
